Patient Registration

This function of the Hospital Management Information System deals with registering new patients, for either OPD or IPD and issuing unique identification numbers to the patients. These numbers are unique throughout the system.

A patient can be registered either at the IPD front office or at the OPD reception. The OPD or IPD identification number is also created for each of the patient's separate visits. This also serves as a part of patient registration. The IPD/OPD ID is used for tracking the medical records of the patient for that particular OPD visit or IPD admission. All medical records of the patient are identified by a combination of numbers, i.e. Patient ID and OPD/IPD ID. The numbers allow for a flexible search in terms of finding the patient's history record.

New patients need to go through the preregistration process which is where the collection of information begins. This process is started before the patient enters the office for an appointment. When a new patient calls for an appointment the information that is usually gathered is the patients full name, telephone number, address, date of birth, gender, reason for the appointment, and if the patient was referred. When the new patient arrives for their appointment they fill out a patient information form or a patient registration form. This form is used to collect demographic information about the patient, such as marital status, social security number, employer, patient health plan, etc. New patients also complete a medical history form. This form asks the patient about their medical history, their family’s medical history, and their social history. Social history is information such as smoking, alcohol use and exercise habits. The physician will go over the medical history form with the patient and the patient’s answers along with the doctor’s notes are documented into the medical record. For new patient’s, that have medical insurance, the insurance card is scanned or photocopies and the information is double checked for accuracy. The new patient’s identification card is also taken and photocopied or scanned…

You probably recognize the picture above from Wikipedia because it is a portrait of the twelve Olympians of the most famous and influential civilization, the Greeks. Yet, the Greeks also have a famous counterpart, the Romans. Both have left their marks on present day culture and have respectfully done so. It is silly, though, to believe that they did not "borrow" some of each others ideas. The Greeks, in fact, were so far ahead that many cultures emulated their findings; the most popular and similar were the Romans. The Romans obtained many of their mythological views, art, and architecture from the Greeks in order to expand their empire.…

Have you ever heard of the book Where The Wild things are by Maurice Sendak? This book is very popular among children and adults. While I was researching and investigating Maurice Sendak, I found a lot of interesting facts about his life and accomplishments. Maurice Sendak was born on June 10, 1928, in Brooklyn, New York and died on May 8, 2012 (About). His Jewish family came to the United States from Poland and lost many members of their extended family due to the Holocaust. Maurice was a sickly child and had to spend a lot of time indoors. He spent a lot of time reading, drawing, and letting his imagination run free (goodreads). Maurice learned to love stories as a young child when his father told him embellished versions of Bible stories. At the age of twelve, Maurice became very inspired by the animated world of Walt Disney’s “Fantasia.” He was fascinated with Disney, particularly Mickey Mouse and the film Fantasia, which he saw when he was twelve and had a lasting effect on him. In 1948 he worked at FAO Schwartz as a window dresser while he took night classes at the New York Arts Students League.…

Medical record numbering and filing is the most important tasks in the management of medical information in health care institutions. Well kept and filed medical records enhance effective and efficient collecting, recording and retrieval of patient health information whenever required. The patient record care system adopted influence the ease of maintenance and retrieval of medical records. According to the Remote Health Branch of United States, health care institutions ought to adopt a standardized medical record numbering and filing systems during storage of valuable medical records. Notably, most health care institutions across the world file and number the medical records numerically basing on the patient’s medical record number. In the recent past, hospitals have numbered and filed patient’s records basing on their names, discharge numbers, or diagnostic code numbers. Some of the commonly used medical records numbering and filing systems include Straight System, Terminal Digit System and Middle Digit System (Terese, 2006).…

Nearly half (47%) of all illegal border crossings into the U.S. occur along the Arizona border with Mexico. As the state with the most illegal crossings t the United States/Mexico border, its remote and dangerous deserts are the entry point for thousands of Mexicans and Central Americans(Cooper). The non-native citizen population has grown rapidly in this area. Arizona had an estimated 460,000 illegal immigrants in April 2010, a figure that had increased fivefold since 1990.(Cooper). Illegal immigration, like most other issues has an upside and a downside. On the upside, there is a steady workforce for the labor industry (agriculture, construction, etc.), and the service industry (restaurants). The agricultural industry in Arizona is comprised of 59% immigrants, the construction industry is made up of 27% workers not native to the United States, and the service industry is populated by 22% foreign born employees. On the downside, illegal immigration brings its share of problems including crime, poverty, uncirculated funds (pay not reintroduced into the economy in which it was originated), and an overburden on social services. It is the latter that sparked the now controversial Senate Bill 1070 that effectively closes the border to all but the few lucky enough to obtain legal status.…
