One day I was ask to transfer Mrs X from bed into wheelchair. Mrs X is paralised so to transfer her a full body hoist is needed. I had to call for help, it wouldn’t be safe to do this by myself. So while I was waiting for the other carer to arrive I have explained to Mrs X what and how we are going to do it. I have also cheked the hoist and battery if its fully charged and operational. I put right size sling on Mrs X with her cooperation. I carefully manovered hoist close to bed, then I hooked sling onto the hoist with short hooks on to of the body and long hooks on bottom. That was when my collegue arrived to help me with transfer. We asked Mrs X if she is ready when she said yes by pushing button on hoist remote I raised Mrs X up, while my collegue was holding her to stop swinging. When she was up I reversed away from bed. When I was in positionfor lowering Mrs X I have opened legs of the hoist. When my collegue manouvered wheelchair into position I was supporting Mrs X on back and knees. When wheelchair was in position my collegue took over of supporting Mrs X and I asked both Mrs X and my collegue if they are ready to lower Mrs X into a wheelchair. After positive answer I have lowered Mrs X into a Wheelchair and after that me and my collegue unhooked sling from the hoist and I safely moved the hoist away. I thanked my collegue for help and I took Mrs X to the dining room.

On explaining our desire to transfer him using the hoist , he declined , stating that he could do it himself ( he has some degree of short term memory loss ) and he asked if he could be assisted to sit up on the edge of the bed and given his zimmer frame ( this is the procedure we followed up to a couple of months before , when we would assist him to do this and he would then be able to stand and walk independently but it came to the point where he found it very difficult to stand and once standing he was very unstable and not able to walk all the way to his chair or transfer to a wheelchair and as we we were not allowed to manually lift him as it would contravene legislation like manual handling operations regulations 1992 and HSE guidelines that basically mean we can only manually lift babies and small children and ensures the safety of ourselves, our colleagues and the person being moved - so assistance and advice was sought from his named Registered Nurse who made a new risk assessment using the procedures and other moving and handling documentation which is in the library and the manuals that come with the hoists and slings e.g. to make sure he had a sling appropriate for his size and weight , this was agreed upon by him and his daughter that he should be transferred using a hoist from now on ) once he was reminded of all this he agreed that it was OK for me and another carer to move him using the hoist and we explained how we would do it . The care plan is checked again to see which hoist is to be used and which size sling should be used to safely transfer him to a wheelchair.…

I work in a nursing home for the dementia of the elderly and I understand that their upbringing and background was very different to how mine was, some of the service users have a very different view on life with their beliefs but I take into account that everyone has their own way of living and thinking for example sadly one of the services users past away last year and her close friend who also lives in the home wanted me to accompany her to her funeral in a church, even though I don’t believe in god I still took the time on my day off to take her and pay my respect and sang the songs throughout the lovely service, religion may affect people in different ways as we are taught differently and brought up in different ways by doing this it made me happy that I made a service user happy that she could attend.it is important that I remember that my personal preferences are different to others and that I can’t and will not try to change that, for example iv recently had a conversation with an elderly woman that I look after about cleanliness, she explained to me that when she was my age she only had a bath once a week and that was only on a Sunday, I explained to her my beliefs in how I have a bath daily and sometimes twice a day as that is how I was brought up, I asked her if she still would like to have a bath on a Sunday but she replied with she doesn’t really mind what day she has her bath but sometimes would like two.…

I work in a mix residential home for the elderly, some of the clients ,Dementia range from mild to severe. The clients are raised in a very different way to how I was raised and it is important that I must remember this and respect their this as we all have personal beliefs and preferences based on our background and upbringing. I have been brought as a Muslim where religious education was compulsory, however we still studied various other religions and were taught to respect all people and there beliefs. Prayers were a daily thing at the end of each assembly and whether or not I wanted to pat attention to the prayer time again I had to respect the people that did. Religion may affect people in many different ways, mainly because we all come from different backgrounds and are taught differently. However, it is important to respect how people honor their religion. I am an atheist however I still make time to talk to my clients about their religion even though my own beliefs it is not some thing that I choose to do as I know it makes the client happy it makes me happy to do this as I know it is some thing that they feel so strongly in and it is amazing at how much I really now look forward talking about it. It is part and parcel of the work that I do and part of showing the clients that we care about what makes them happy and respect their beliefs even if ours are different.…

I started my shift at 7:15 am. My work clothes are comfortable and unrestrictive; my footwear is flat, sensible and offers protection to my feet and ankles. When I arrive to the service user’s home, I knock on the door and wait to be let in either by one of the service user’s or by a work colleague. Once inside I greet the service user, if they are awake. I then head to the sleep in room, to start verbal handover with my work colleague. After the 15 minute handover, I then check and sign the handover sheet. Check the on call rota, should there be an incident or or accident. Also check the communication book and also check any money and medication, then checking the activity planner. Once the service user wakes, we say hello to each other, and chat for a short period. I encourage them to start their personal care. After, I ask them if they need any support with getting dressed and preparing breakast. The time is now 8:00 so I administrer the morning medication. I always wash my hands before handling food or medication and after assisting with any personal care. After the service user has had their breakfast. I support them with the washing up. I will then encourage the service user to clean their property. I will then support the indiviual in safely cleaning, all ares with correct products. After this I consult with the service user, about their activity plan for the day, if the service user decided to go into town. I ask them how they would prefer to travel, by bus or by personal car.…

When I started my work placement at my setting I was working with my members of staff and manager who told me how to monitor children when they eat and which child cannot eat certain food because of their religion or culture. Before I started to give out the food for the children I had to read the food policies and procedures to know how to serve the food and drink for the children. At my placement I learnt how to monitor the children when they ate and how much they ate, and also about their allergies. I had to monitor how much the children drink and I had to make sure that they drink enough water or milk.…

Mary (changed name) was admitted with right cerebrovascular accident (CVA) and was now just waiting for residential placement. One of the side effects left over from her stroke was that she had a drop-foot for which she had a splint and a walking stick; she had declined to use a frame. Mary was one of the patients on my team so I was getting to know her quite well during my first two weeks on the ward. I was working with another patient when I saw Mary twist on her ankle and try to steady herself on her stick. This had the effect of spinning her round and she fell to the floor onto her left hip. I ran to help, she was in a lot of pain and slight shock, I called down the ward for help several members of staff came to help saying "do this" and "do that" all at the same time, I was quite over whelmed. Mary was rolled back and forth onto a sling, hoisted up and placed on her bed, all the while I was looking after her head, I did not know what else to do so I talked to her to calm her down, which was difficult due to all the commotion. On initial examination the senior nurse on duty said it looked like she had broken her femur, which was later confirmed. Her residential placement was cancelled and she was transferred to stepping hill.…
