CM traveled to consumer’s home for the purpose of monitoring and assessing needs. CM arrived at consumer home and met with consumer. CM asked consumer how she has been doing. Consumer stated she has not been doing well. Consumer said, “I told my daughter that I am no longer signing any papers from Alta Pointe until I know what service Alta Pointe provides.” CM explained to consumer that Alta Pointe still provides the same service. CM and consumer discussed consumer medication regimen. Consumer stated she is no longer taking her medication because the medication is causing her to have a rash. Consumer was pointing at her skin and stated she had a rash on her neck and arm. CM suspect consumer is delusional at current time as evidenced consumer
