A hazard is things that can cause harm to someone. A lot of hazards are potential dangers that can be avoided.
Dementia is a chronic or persistent disorder of the mental processes caused by brain disease or injury and marked by memory disorders, personality changes, and impaired reasoning. (https://www.google.co.uk/#q=definition+of+dementia, 20/1/14). Dementia is a progressive disease which means that it gets worse.
Hazards and potential harms for service users with dementia:
Hazard
Potential harm
Fire exit door left open, and the service user with dementia walks through the fire exit onto the main roads.
Service user could have tripped over the pavement and fallen and broken her hip. Service user could have wandered into the main road and could get hit by a car. The loud noises from the road could also cause the service user more confusion.
Kitchen left unsupervised so service user could walk into the kitchen and gain access to dangerous utensils, such as sharp knives, pots and pans.
Service user could fall if the kitchen has slippery floor and break her back. Service user could use the knives to cut themselves or to possibly hurt another service user or member of staff.
Service user left unsupervised to come from upstairs to downstairs.
Service user could fall down the stairs and break her hip, neck or back which can make the service user paralysed.

Arthritis is a disease causing painful inflammation and stiffness of the joints. (https://www.google.co.uk/#q=Arthritis+definition, 20/1/14).

Service user holding a hot cup of tea, their joints start to ache and drop the cup on their lap.
Service user could burn themselves from the hot tea and could smash the cup on the floor, which could lead to them cutting themselves.
