Summative Assessment
Whilst I attended clinical placement at xxx Nursing Home, I was asked to accompany the registered nurse (RN) attending zzz Ward, which houses people who have dementia and other similar conditions. During handover, we were advised about an ongoing issue with two of the residents; a married female named Joan (not her real name) and an unmarried man called Barry (not his real name). Barry had been visiting Joan in her room, and making sexual advances to her throughout the day. Both of these residents suffered from severe dementia and were not fully aware of their actions; however, Barry seemed to show intelligence with his behaviour and would wait until the staff were busy with other residents before attempting to visit Joan in her room. This posed a problem to staff as generally there was only one staff member on duty in this ward. The other issue staff were presented with was Joan’s forgetfulness that she is married, and due to that reason she made no attempt to deter Barry. Joan’s husband, who resided upstairs in low care and was of sound mind, had visited Joan during the day and witnessed Barry leaving Joan’s room and had noticed the attention that was being given to his wife from the other resident. He was extremely upset and contacted his son by telephone in a panic due to the situation.
Approximately forty-five minutes later, I was alone in the nurse’s office when a burly, older man entered the room and introduced himself as the son of Joan and her husband. He stated his reason for being there and understandably, seemed extremely upset at the current situation with his parents and Barry. I explained to him that as a student nurse, I had to get the RN on duty, who could then discuss the situation with him (ANMC Competencies 1.1, 1.2, 1.3, 2.5, 7.4, 7.5). After locating the RN, I described the details of the brief meeting with the man in the office and asked her to come and speak with him. After a long discussion between the son and the RN and myself, there

Susan and I had a nurse-patient relationship that's based on mutual trust and respect. I had been providing care in a manner that enables Susan to be an equal partner in achieving wellness. I had always make sure Susan has privacy when provide care and be sure that her basic needs are met, including relieving pain or other sources of discomfort. I too had actively listened to her to make sure I understand her concerns by restating what she has verbalized. I had maintained professional boundaries like respecting differences in her cultures. We as nurses help Susan achieve harmony in mind, body, and spirit when engaging in…

When working in the care industry whether that is with the elderly, mental health or children it becomes apparent that you as an individual become empowered by virtue of being primary care for that specific individual hence being an influential figure in any individual’s life recovery and rehabilitation. Due to this factor we have had many horrific incidents which have involved individuals in a position of power and whom have abused this trust and disregarded legislation, policy and procedure to continually systematically abuse service users in a vulnerable position. It is no secret that this has happened and still continues today within places of care. This is the purpose of the new legislative law updated and reviewed to combat and eliminate such barbaric behaviours displayed by so called supportive staff members in supposedly places of secure safe environments. The purpose of legislative law is to filter new and reform existing legislation through to the relevant policies and procedures within specific places of work. So to summarise the purpose of legislation in terms of managing difficult behaviours is that it functions to safe guard the vulnerability of service users in places of care which predominantly gets feed down through governing bodies to the policies and procedures file within the office within your place of work. It can be looked upon as a code of good practice with regards how to deal with vulnerable adults whom display challenging behaviours within places of care. When working within such an environment it is almost expected to receive challenging behaviour due to the different backgrounds and diagnosis the service users have so it would not be acceptable to react within such a way as you would when away from the unit due to the very essence of the clientele we deal with. This is not to say we are not in a vulnerable position to…

Offering inappropriate treatment or care is unacceptable within a care setting because every resident deserves to be treated equally and professionally. As a care worker it is your duty to ensure that they are receiving the best care possible no matter why they are in the situation they are in. A resident’s past should not affect the way you treat or think of them, we are there to support and promote independence for all residents. If a resident is showing unusual behaviour you should try to find out what is wrong with that resident and don’t label them as being awkward or doing it for attention.…

As stated before, I decided to do my participant observation exercise in the nursing home during the evening where the residents are getting ready to go to supper and come back and go to bed. At 4:30, the residents were just getting back from chapel and needed to use the restroom to go to supper, which started at 5:30. Many residents had their lights on and wanted to be the first ones to be attended. I knew this as I walked down the hallway I could hear some residents tell the CNAs to help them. During this time, the nurses were working on passing the medicine and charting other things. At 5:30, the residents wanted to go to supper as soon as possible so that they would not be late. When everyone was at the dining room, the CNAs had to help feed those who could not feed themselves. by 6:30, everyone wanted to be out of the dining room to go to bed. During this time, the nurses were once again passing medicine to those who get it after meals. The CNAs once again had many lights to answer as fast as they could to ensure that the resident was happy and pleased with their service. During this time, resident 1 had her light on and when it was explained to her that those who were at a risk of falling if unattended had to be attended first. She got upset and said that…

However, my client has the ability to take actions by telling my client’s mother on what happened with a few words, sad emotion (crying) and demonstrating what happened with his hands. Although my client can’t speak or talk effectively, the carer can set up a few cameras that will record everything at each corner of the building my client is been taken care of by the carer; my client’s carer can see what’s happening when my client is been taken care of by the carer; my client’s carer can tell the police and charge the carer with the recorded video footage of the camera to the police. Furthermore, my client’s carer can give the carer a warning or fire them from their job, which is unlikely, as the client has violated the Code of Practise; the carer should be charged (like they can’t work as a care assistant), as they shouldn’t be taking care of other…

A nurse named Bilquees Ahktar abused elderly residents in a Lancashire care home through breaching their rights. Ms Ahktar is facing twelve charges of misconduct in 2010. She did a variety of unacceptable behaviour whilst she was taking care of the elderly residents. One of the things she has done before was trying to force medication into a male patient’s mouth which already clearly shows that she is taking away the right of the elderly from having choices and preferences in terms of medication. The incident also proves that Ms Ahktar is not giving the elderly the right to be respected. This is only one of the misbehaviours she…

I was on my third day of residential care placement; the staff had just started to take turns for their morning tea break so I took the time to catch up on my case study patient’s medical history in the nurses’ station. Within a few minutes the Manager of the rest home ran in to gather the blood pressure machine and bandages. She informed another student nurse and myself to “take these to Max’s (pseudonym) room NOW, while I call an ambulance”.…

My preceptor had explained to me the process involved in care planning for a patient on the unit, the doctor will do the majority of the assessment, the nurse carries out the risk assessment and completes Roper Logan and Tierney nursing assessment which is the nursing model used by the Louth/Meath services. The nurse also carries out an admission checklist. When the patient has been admitted and the nurse has gathered all the relevant information they will then incorporate the care plan. I familiarised myself with the documents, I will admit I felt a little apprehensive; I was worried I might say the wrong thing or ask the wrong question. As John had a previous history, I had been informed that he suffers severe paranoid delusions. This immediately alarmed me in the sense of communication difficulties.…

If I was faces with this situation I would first talk to the nurses who were humiliating the patients. While confronting the nurses I would make sure that they understand why patients especially the elderly should be treated with respect. By using the tradition approach, I can appeal to their beliefs about how people should be cared for. This approach may not be effective if the nurses to not see the situation the way everyone else does. The authority approach would be more effective in this situation. The nurses are more likely to listen to a person with authority then someone who has only been working at the facility for a week. However, if the person with authority does not act on the situation, or is not properly educated about the…
