Introduction
Unique artificial airways and airway devices have been developed to solve a variety of diverse clinical problems.
Since the early 1960s when plastics replaced rubber in the manufacture of endotracheal tubes (ETs), thousands of individual airways have been designed and produced. Unbridled imagination and a creative spirit have led to the invention of a variety of devices, many of which have the potential for patient harm. The problem of tissue toxicity of the materials used and the need to connect to other respiratory devices and anesthesia devices necessitated some common standards to which all devices must conform. These concerns of compatibility among the various designs have been minimized by development and adoption of consensus standards within the American Society for Testing and Materials (ASTM) (Internet address: http:// www.astm.org) and the American National Standards Institute (ANSI).
The original ASTM technical subcommittee dealing with anesthesia and airway equipment was designated the Z-79
Committee. This has now been replaced with the F-29

    The purpose of this specific study is to compare the neuromuscular block reversal rates and effects for sugammadex, a selective relaxant binding agent, with neostigmine. In many types of surgery, there appears to be a benefit for the patient to be kept in a state of profound relaxation throughout the entire procedure.     ABSTRACT Endotracheal tubes (ETT), are used to maintain patient airway access during surgical procedures. The design presented here addresses a current need of the gold standard ETT by allowing for the passage of a larger bronchoscope through the inner lumen of the ETT while keeping outer dimensions constant. This design incorporates a flexible membrane with a wavy geometry to ensure a return to the neutral axis, thus allowing for even ventilation of both lungs. Flow tests simulating surgical ventilation conditions were conducted. These results confirmed that adequate air flow could be obtained independent of any pressure differences across the membrane. Tests of the prototype also confirmed that maximum instrumentation diameter was increased through this design when compared to a commercially available ETT with identical outer dimensions (35 Fr). Finite element analysis was conducted to determine the necessary thickness of the membrane for a safe design. One key design feature of the membrane is that its state of maximum deflection (2.1mm) does not significantly hinder airflow through the smaller lumen. Tests to further evaluate the functional requirements of safety and ease of use are currently under way. Methods of manufacturing this design are also under investigation.     Indications for bronchial hygiene therapy for acute conditions are copious secretions 25-30 ml/day, acute respiratory failure with retained secretions, acute lobar atelectasis, and V/Q abnormalities caused by unilateral lung disease. Chronic conditions that indicate therapy are cystic fibrosis, bronchiectasis, ciliary dyskinetic syndromes, and chronic bronchitis. First we must assess the need for bronchial hygiene therapy. We do this by looking at the patient’s history, presence of artificial airway, CXR, results of PFT, age, and any type of surgery the patient has under gone. We also look at the patient’s appearance such as posture, breathing pattern, vital signs, general physical appearance. There are five general approaches to bronchial hygiene therapy. The first approach is postural drainage. Postural drainage uses gravity to clear retained secretions there are positions the patient can be put in to effectively drain secrections. Some positions are for anterior upper segments the patient is sitting up with a pillow under their knees. A second position is right lateral segment the patient is lying on their left side with their legs elevated eighteen inches.
