Dr. House, my name is Jet, I am the nurse caring for Mrs. Cotton in Riverway wing room number 337
S - I received an order for MRI, however she has elevated creatinine level and I would like to clarify the order with you
B - Mrs. Cotton came with a chief complain of excruciating chest pain and dyspnea and
MRI was ordered to rule out the reason for increased intracranial pressure
A - Her creatinine level is 2.9, not in the normal range allowable for any radiologic examinations R - I think that we can proceed with an MRI after his creatinine level is back to normal range Scenario 2
S - Dr. Holmes this is Jet, a nurse from Med/Surg at CHCMH. I’m calling about Mrs.
Lara’s respiratory status. She started to get shortness of breath about 30 minutes ago.
Her vital signs are normal except RR: 26 breaths per minute
B - Mrs Cotton is a 23 year old who undergone subtotal thyroidectomy yesterday and has no history of respiratory or heart problems. She is alert, and oriented to place and time, and complaining of sudden difficulty of breathing.
A - The patient is in orthopneic position, with hoarse voice, no pain reported. Vital signs are as follows: BP: 130/90, HR: 88 bpm, strong on all pulse sites, capillary refill of 3 seconds. RR: 26 breaths/minute, no adventitious sounds heard upon auscultation, O2 saturation: 96%, Temp: 36.9˚C by tympanic. Skin is dry and cold to touch. Im not sure whats the problem is but I wonder if she is having laryngeal spasm which causes airway obstruction, her vital signs except her RR are within normal range however it might become unstable later on and we need to do something.
R - I would like you to come and see the patient immediately and we start her on
Oxygen therapy stat? Are there any exams or labs you like to order? let me repeat back what you have said:
-oxygen by facemask to keep oxygen saturation about 96%, keep tracheostomy tray at bedside and you will be up to see
