Dear Ms. Odiver:

This is to request your good office to accommodate the following Bachelor of Science in Management Accounting student as intern in your company: Go , Monica In your assignment of tasks, our practicum student is expected to conform to the following:

1. She is required to report for work for a total of 200 hours for the duration of the practicum period.
2. She is expected to abide by the rules and regulations in your company during the practicum period.
3. The company reserves the right to terminate the practicum work for a reason of misconduct or for reasons that may render the student unfit for practicum work.
4. She shall not be entitled to any benefit whatsoever, unless such benefit shall be given by your company voluntarily.
5. She should not be given functions related to physical cash handling and cash custodianship.
6. Your firm shall not be held responsible for any claims or benefits arising from any injuries or accidents that the student-apprentice shall suffer in the course of the apprenticeship program if such injury occurred within your company premises, otherwise you shall be held liable.

We would appreciate if our practicumer/intern is given the opportunity to be exposed to Accounting, Audit, Taxation and other related tasks. At the end of the practicum period, may we also request you to provide a certification for each student indicating:

No. of hours attended (Daily Time Record)
Over-all performance assessment of the student (an evaluation form will be provided)
The practicum duty will officially start as per your instruction. The student may be allowed to extend her duty hours at the discretion of the student when ever her services are still needed and as long as it will not be in
