Mr. Daniels Narrative Report
Mr. Daniels completed a One Day Vocational Evaluation on 09/12/2017 at the WIN Job Center in Carthage. He was on time and dressed appropriately. He walks with a slight limp. He was polite and focused during the intake. His speech pattern and thought process appeared appropriate and normal. He had no apparent trouble filling out paperwork or completing assessments. He appeared to do his best on all assessments. Mr. Daniels stated that his disability is obesity and osteoarthritis. However, he exclaimed, “I have lost 40 lbs.” When asked, “How would your disability affect your ability to work?” Mr. Daniels stated that “I can only stand up for about 1 ½ hours working.     Murray’s physical capacity is restricted to sedentary work that involves lifting up to 10 pounds at a time, occasionally, and requires occasional walking and standing, for up to 2 hours a day (he cannot stand for about 3 hours without experiencing discomfort). To avoid pain exacerbation, Clayton is recommended to refrain from performing job duties requiring long hours of standing, walking, lifting and carrying heavy objects. I arrived at 1430 Cottonwood Dr and spoke to the complainant, who stated that the dog has been running loose around the area and almost getting hit by cars.
