Discrepancies found in a 24 hour period with Humalog Insulin
An audit was performed during the time period of November 5 at 11:22 am to November 6 at 11:12. This audit was conducted due the frequency of Humalog insulin vials found outside of the Med Dispense machine. Nine occurrences where found where nurses charted Humalog insulin as given, but not recorded as being taken from the Med dispense. One occurrence, a nurse recorded a dispense value of 1 unit from the Med dispense, but actually charted 12 units give to the patient.
There was a total amount of 31 units of Humalog insulin within a 24 hour period that was charted as given to patients, but not recorded as dispensed from the Med Dispense.
