SUBJECTIVE Mr. Hamilton was readmitted to MCCRC on 03/02 and seen on 03/05/2018. Mr. Hamilton had been here after a motor vehicle accident, leaving here with fractured ribs, pneumonitis, fractured sternal, and a complicated right acetabular hip fracture. On May 24, 1992, Ramon Flores was working at a gas station that was getting robbed and was shot and killed. The man charged with his murder was Michael R. Pulido. He was charged with first degree murder along the felony murder law. That law states that killing during the act of a felony, in this case a robbery, is automatically first degree murder. The way I have interpreted the facts of this case, the defendant was the one who pulled the trigger and point toward him as the killer. To come to this decision, the jury had to conclude that the defendant had intent to commit a felony, killed during the act of a dangerous felony, and that the victims death was a foreseeable outcome of the felony.…

Dave Sharland – Mr. Sharland a physical therapist for the North Florida/South Georgia Veteran’s Administration (VA) reported that during his career, he has participated in over 800 physical therapy visits in home settings and been very positive. The Council questioned Mr. Sharland on licensure requirements, technical platform, types of services offered, and patient and patient representative access to services. Mr. Sharland to explain the health care providers working with the VA are only required to obtain a license in one state, but they are limited to providing services to only VA patients. Paul Kruse grew up in a family that had limited means. When Kruse reached college, he studied theology and became a youth minister. Approximately 2am, Tobin was coming downstairs alongside Pheonix from their room. Tobin lost balance on staircase, and then tripped. At this time, Pheonix tried to help Tobin regain her balance, but unsuccessful. At this time, Tobin struggled to catch grip using both hands, as a result of this, her left index finger got broken. Writer instantly called 911 requested for EMS. While waiting for EMS, writer asked Tobin if they were planning to go somewhere before the incident, she told writer that, they were going to Metro to get a drink. On April 23, 2015, at 11:58 a.m., I spoke with Jerry Cummings who is a medical practice consultant. Cummings explained that he serves as a medical consultant and helps physicians manage their practices as well as hospitals. Cummings told me he is aware of the allegations made by Amy Morgan and assumes they are true, but doesn't know firsthand. He said that Harrison relies upon him for a lot of things and he has been under the impression that Harrison probably needs to retire. He has also discussed the allegations with Harrison who denies having any type of drinking problem. He requested Harrison not perform any procedures until he gets evaluated. 1. Billy is a voluntary patient and can leave the ward at any time. He has attempted to commit suicide more than once. He is deathly afraid of his mother and authority figures. I think that Billy has Anxiety disorder as well as social phobia. I do not think that Billy should be in the ward, he would do better at a rehabilitation facility. The ward is for insane or criminally insane and Billy does not belong there.…
