Investigation of a Workplace Incident
Investigation Policy
PURPOSE

The purpose of this policy is to ensure that: all hazards, near misses and injuries are reported to management. investigations are carried out in a sensible manner with preventative actions identified and put into practice in due course. follow up monitoring is undertaken to ensure hazards are removed or reduced as much as possible and that no new hazards will be introduced to the workplace as a result. accurate information is collected to investigate trends in incident types and offer suggestions and recommendations with the aim of controlling hazards. all information is available swiftly to a variety of groups in the workplace
SCOPE

The policy applies to: the reporting of all incidents, near misses / incidents in the workplace the reporting of dangerous occurrences resulting in damage to equipment, machinery, property, environment or facilities.
The reason behind the workplace notification of an incident is to keep employees, supervisors and managers informed of the facts surrounding the incident.
A number of procedures may follow as a result of an incident e.g. measures put in place on other sites or an independent investigation launched for the incident.
A suggested flow chart is illustrated below:

RESPONSIBILITIES

Higher Management shall:
Implement the investigation policy within the workplace
Develop appropriate reporting processes to ensure timely reporting of incident / hazard
Ensure that adequate financial provision is available to put into practice, corrective or preventive actions identified from incident (hazard) report investigations.
Ensure that all staff receive incident / hazard investigation training
Managers/Supervisors shall:
Ensure that local procedures are developed and that they are followed all staff
Ensure all staff and students are aware of the purpose of reporting hazards, near

    An incident report must be delivered to the Office for Research Safety within five days of the incident. Reports are reviewed by the Chemical and Biological Safety Committee, or the Radiation Safety Committee.  An incident is defined as any unplanned and unwanted event that occurred during the performance of work activities and that resulted in or could have led to injury or material damage to property. Incident repercussions range from minor (e.g., a broken mercury thermometer) to significant (e.g., a 5-gallon bottle of sulfuric acid dropped in a heavy-traffic hallway).  An Incident Report is appropriate for “near misses,” incidents not resulting in personal harm or property damage, but which might have, under slightly different circumstances.  The Incident Report requires responses from (I) the person involved, (II) any witnesses to the incident, and (III) the Principal Investigator/Supervisor. Attach additional pages if necessary to complete the report. Reports that are not signed by the PI/supervisor will be returned for completion. The committees require input from the supervisor. See completed Example following the instructions.  Commonly, there are multiple causes in any given incident—all of which should be identified. Provide a complete and detailed response to each question, making a serious attempt to identify all “root cause(s).” The contributing factors were probably evident, but overlooked or unrecognized previously. These factors become more distinctly identifiable in light of the specifics of the incident A well-planned work process will include multiple layers of safeguards. Once causes are identified at all levels, consider safeguards and procedures that might be changed to prevent future incidents. .…
