An Electronic Health Record (EHR) is an electronic version of a patients paper chart. The EHR stores the same data that you would file in a paper chart. The EHR includes the following: demographics‚ progress notes‚ problems‚ medication list‚ vital signs‚ past medical history‚ immunizations‚ laboratory data and radiology reports.
