Page 1 Compliance plans are in place to insure the medical facilities are following any official requirements. Some areas which are included in a compliance plan are staff education and training, early detection of problems in the facility, and avoidance of negative legal actions. The steps in the medical billing process that are covered in a compliance plan are steps five reviewing coding compliance and step six check billing compliance. Medical records should follow a compliance plan to insure all areas of patient records are complete. Medical records apply to all steps of the billing process. These areas include record accuracy, patient’s condition and diagnoses, the patient’s course of care should be outlined. Medical records must be accurately completed and kept up to date with the patient’s current information. Patient records are private legal documents and in order to avoid any legal actions the compliance plan should be followed. All steps of the billing process apply to documentations standards and should follow a compliance plan. To insure all patient and facility records follow the set documentation standards employees should be educated in the correct areas of completion of medical and facility records to decrease the case of documentation errors. Employees must follow all documentation standards to ensure correct information is input into patient records which aids in the process of accuracy, etiquette, and customer service. Ensuring all standards and plans are followed allows for lower chances of record error and or medical lawsuits.     The ten steps to the medical billing process can be categorized into three main groups. These three groups are the visit, claims, and post-claims processes. When it comes to the visit category, this category has a great deal to do with the patients personal information getting the patient established, such as pre-registering the patient. This is the process when we use the HIPPA privacy act to help gain the trust and confidence of the new patients and returning patients to our office. Also, we have the step that we take to inform the patient of his or her obligations to pay this is another form of the HIPPA act that we must follow when it comes to the patient’s personal information such as finances. The claim category uses the help of the ICD, CPT, and the HCPCS billing strategies to help ensure that we are doing all that we can to fully utilize the patients insurance and to better suit the patient. When using these forms we are reviewing the coding compliances. We are also checking billing compliances, when we are made responsible for billing the correct information and codes to the insurances that are involved. In the claim category we are also made aware that we must follow any rules that the proper billing and coding standards set for us. Preparing and submitting the claim is the key to success in any office this is why this process is so important to follow. The last category of the steps that we come to is post- claims where monitoring payer adjustments, generate patient statements, and follow up with patients payments along with any collections. This step brings us back to following any HIPPA regulations when it comes to collecting payment and gathering information for collections. This privacy act must be acknowledged at all times and followed to ensure that you get paid and the patients can not complain.     The submittal of claims to insurance companies requesting payment for medical services provided by a doctor to a patient is called the medical billing process. Ten steps make up the process: preregistration of patients; establishment of financial responsibility for the visit; checking patients in; checking patients out; the review of coding compliance; verifying billing compliance; the preparation and transmittal of claims; the monitoring of payer adjudication; generation of patient statements; and the follow-up of payments by the patients and the handling of collections. HCPCS, HIPAA, CPT, and ICD have an influence on every step of the process. The 9th Revision-Clinical Modification (ICD-9-CM) is a global categorization of disease and contains sets of codes. These codes give information for evenly measures and diagnoses. The ICD-9 code has three digits, and these three may be followed by a decimal point and then two more digits. The Healthcare Common procedure coding system (HCPCS) does not give diagnosis information, only information about the procedure area. The purpose of HCPCS codes is to process hospital treatments for outpatient services. Physicians also use these codes. ICD-9 procedure codes are required by HIPAA for their porting procedures of hospital inpatients. The numerical codes for CPT and the diagnoses areas signed by the coding team.
