I like that you discussed about Synapse. One of the issues that I have encountered with Synapse was Psychiatry and Community Mental Health & Addictions nursing staff are the only ones who have access to it. It would be nice to see that Emergency staff have access to Synapse when clients present to these areas for medical help. For the patients who frequent at the Emergency department at Mills Memorial Hospital for mental health reasons had a client centered care plan developed from either the community clinicians or inpatient psychiatric staff that is placed into a binder in ER department for future reference. For example, a community clinician worked with a client, who was diagnosed with paranoid schizophrenia, who frequently shows up emergency

    After observation of handling the care of Ms.R by Ms.W the need for a plan to promote interdisciplinary care/communication and the maximum utilization of it, would definitely be an important process that requires intervention. One of the steps that I, as the new nurse supervisor would implement, will be to set up a daily stand up meeting where all the disciplines are involved for at least 30-60 minutes where we discuss and go over all the patients and make notes on what the pane for their treatments are. Given the situation, an end of the day meeting would probably be more efficient where all the disciplines can address the issues that they had during the previous day and discuss into determining goals that would be available to work towards them by the beginning of the next day or with the new shift staff. This will be an almost immediate intervention and would start addressing the issues at the earliest most convenient time. During the meeting, it will be mandatory where the discipline in charge of their patient(s) will have to disclose the steps taken so far, goals to be achieved, interventions planned and the time frame they strive to and/or plan to achieve these goals. Since all the disciplines will be present in the meeting, it will be a good tool to utilize an opinion sharing and collegiality experience to help in suggesting, advising and offering an intervention that might be beneficial and result in positive outcomes for the patient(s) that will be discussed on. Another step that I will suggest implementing would be a documented form on each note that needs to be filled out daily where questions such as:…

    Watching the documentary on the mental health patients at Bellevue makes me kind of uneasy. It makes you understand that people with mental disabilities have a real problem. I feel as if a lot of people believe that people make themselves sick. Which if you watch this documentary you understand that no one in there right mind would do this kind of stuff on purpose. The reason they go to the hospital usually is because most of them don’t know how to get actual help so they act out trying to get help where they cant seem to find it anywhere else. I feel for the doctors that are at Bellevue trying to help out because it must be a mentatally draining job. A lot of the patients seemed to have an alocohol and drug issue. I believe addiction makes it harder to actually see that you need help. The drugs and alcohol seem to mask the problems they actually have so people just usually assume they are just regular ol’ junkies. The patients seem uneasy a lot of the time. The doctors try their best to help but when a patient is screaming at the top of their lungs at you and you cant really do much to help them because they’re not telling you whats wrong they must feel just as miserable as the patient. If I was a doctor and I wasn’t able to help a patient that really needed help I would probably feel worthless. They don’t have a huge supply of psychiatrists there though, a lot of the patients can only see their psychiatrists once a week which makes them frustrated. Which usually makes them want to leave and if they do leave the patients are usually back in a couple of days. It seems as if a lot of the patients don’t have any means to go seek treatment anywhere else. There was a couple homeless people and people who just don’t have the money to go see a therapist or a psychiatrist. I know it seems like a stretch but don’t you think if we had free open clinics for people with mental issues there would be less suicides a year? Mental illness seems to be…
