Summary table of the National Cancer Institute (NCI) Common Terminology Criteria for Adverse Events (CTCAE) version 3.0 used for Assessing Oral Mucositis:

Grade Symptoms (Clinical Scale)
Comments (Functional Scale)

Grade 1
Painless Ulcers and mild oral soreness with Erythema
Minimal Symptoms
1. Normal diet tolerable
2. Minimum respiratory symptoms

Grade 2
Painful Erythema, Ulcers present but can eat and swallow
1. Symptomatic but can eat and swallow modified diet.
2. Symptoms not badly interfering with activity of daily living

Grade 3
Painful Erythema, Oedema, Oral Ulcers are present. Patient only able to ingest liquids or may require IV hydration
1. Symptomatic and Unable to adequately

    HISTORY OF PRESENT ILLNESS: This patient is a 57-year-old Cuban woman with a long history of rheumatoid arthritis. She has received methotrexate on a weekly basis as an outpatient for many years. Approximately 2 weeks ago she developed a respiratory infection for which she received antibiotics and completed that course of antibiotics. She developed some ulcerations of her mouth and was instructed to discontinue the methotrexate approximately 10 days ago.
