DATA: Nina (middle-late 20’s) woman who is single is a professional ballet dancer at a dance company that lives at home with her single mother. PP/CC: Nina is having major stress after being selected the lead role. She is having delusions and hallucinations on a regular basis. She has the mindset that she has to be perfect for the performance. SS: Nina is having persecutory delusions, visual/auditory hallucinations, self-harm, and impulsive behavior. She reports seeing things almost daily. Constantly scratches her back to the point of bleeding but typically blacks out and doesn’t remember doing it. Recently has been promiscuous (i.e. going out and drinking/having sex). Has few or no friends and is very close to her mother. Seems to have many anger issues (i.e. lashing out on mother and other dancers).
BPS: Nina presents biologically unhealthy. Has lost 5 lbs the last month and has been forcing herself to puke. Patient is a white, English speaking American. She is in physical shape and doesn’t report taking any medication for mental health issues. Graduated high school and completed 1 semester of college before dropping out due to social issues. Lives in an apartment with her mother, Erica, who reports being a single mother. Mother presents signs of obsessive behavior and is very unstable. Mother is also physically in shape stated she used to be a ballet dancer before Nina was born. Mother treats Nina as if she is still a child and Nina has signs of playing into it by calling her “mommy.” Nina reports still having stuffed animals in her room. Mother helps her undress/clips her nails for her on a regular basis. No history of taking medication in the past. Nina presents as a heterosexual female but has …show more content…
At times, has not presented Ox4. Her dress is age/weather appropriate. Presents with athletic build but has recently lost weight. Has some abnormal movements (i.e. scratching her back to point of bleeding when stressed). Nina has appropriate affect especially when feeling sad or stressed. Also has appropriate mood, shows she is ecstatic when she gets the lead role. Memory intact x3. Is able to easily remember the dances she is taught. Nina has normal vocabulary.
