SPACE SHUTTLE CHALLENGER ACCIDENT INVESTIGATION
Introduction:
The space shuttle challenger took away the life of 7 crew members on January 28, 1986. The spacecraft was broke up into small parts on the Atlantic Ocean and the banks of central Florida on US time 11:39 am. The reason behind the disintegration was the O-Ring seal in the right solid rocket booster failed at the liftoff of the shuttle.The failure caused a breach in the joint it sealed,allowing the high-pressure gas from the solid rocket motor to reach outside and attack upon the booster attachment hard wire and the fuel tank. The hotel was equipped to conduct an open concept atrium,conference hall and more with 40 story hotel tower. Inside the architecture, the second,third and fourth floor was connected with three walkways and it was 50 feet height,17 feet width and 145 feet long. The project was initiated on 1976 with Gollum-Colaco International Incorporation (G.C.E) as the structural engineering team and the construction was begun in spring of 1978 and in early December the project started to erect the atrium. By the following February, Havens steel company modified the design on connection rod of second and fourth floors walkway from a single rod to double rod to enhance the strength. On the consecutive October during the construction, the part of atrium collapsed and an inspection team was made for the investigation regarding the incident and G.C.E cleared all the steel connections at the premise and it was open on 1980 to public. After that on 1981,July 17 at an eve the hotel witnessed a loud crack at second and fourth floor walkways and it crashed down to the ground floor.There were about 2000 at the premise for a dance competition, in that 114 were put to death and more than 200 were severely injured.The failure occurred because of the several reasons and with a combination of things and the most thing was the design of walkway.The proposed design was
