You have two great examples of incidents that tie into risk management. The country club should have definitely had more signs up indicating that the diving board was broken. They also should have had it blocked off or a barricade that would not allow people to climb on it. Maybe for future references since the country club knows that some people will not follow the signs, they should have a security guard keep an eye on it and let patrons know it is not available for use.
