Dear Dr. Gan
TRANFER OF PATIENT TO BOAS CLINIC
Refer to the matter above, on the 25th of March, one of my patient named Ms Goh Li Wei will be transferred to BOAS home. As this patient had been treated by me for a long time, I would like to provide some information and backgrounds of her to you. With that, work would definitely become easier when she is at BOAS home.

Miss Goh is now 53 years old. She was admitted into Sime Darby Medical Centre last year on the 15th of December 2012 due to serious fracture of the ankle bone. The cause of this injury was that she fell from the stairs in a hypermarket. In the past 2 months, she had been treated by me and now becoming better where palpation on the injury area would cause pain anymore. However, she is still having difficulties in walking where there is weakness on the ankle joint caused by the injury. Since BOAS Home is having a well-known physiotherapist and Ms Goh has no family member to take care of her, therefore, she will be sent to BOAS Home to have treatment for few months.

Ms Goh has a normal mental state. One thing about her attitude is that she is a hot tempered person and she gets frustrated easily when she does not get what she wants or needs in time. For example, food is preferably sent to her on time. Besides, she has Parkinson’s disease. Every time after meal, medicine has to be given to her. She has no vision problem but a little hearing loss. So I would like to advice the physiotherapists in-charge to raise their voices when they are giving instructions to Ms Goh during treatment time. In addition, she has allergy to prawn. Therefore, it is better that her meal should not contain any prawn.

If you have any further questions, please call my office at (555) 555-4272.

Yours truly

Dr. Tan Kar

I have been working for a while in the Nursing Home and I have met patients with different situations. We have been dealing with more Nursing residents but getting to know with Dementia patients is very interesting. I have met Mrs. E which had been discharged from the hospital. She couldn’t communicate properly on what she wants, even her family are having a problem to recognize the things she liked and wants. It was a challenge for us to help her in her daily personal hygiene and especially eating and drinking. When she arrived we tried to comfort her and made comfortable. Her family was very supportive in her care but it’s very hard to predict what she really wants. We checked her weight, as the process of checking if what they need for their nutrition. Mrs. E had been spitting out food at meal times so we have tried to give her sweet drinks to divert her attention with the food. During the past days we have been trying to identify on how to communicate with her through different ways until we tried to hold her hands while eating to keep her calm.…

and Mrs. Thomas will need to play an active role in promoting Mrs. Thomas’ health. Although her health will steadily decline, she needs to feel as independent as possible for as long as possible. As previously discussed, pain management will be key to improving Mrs. Thomas’ functional ability. Oppositely, while the relief of pain will always add to the facilitation of functional ability, side effects of many pain medications can have an adverse affect on functional ability. Too many or the wrong kind can be unsafe as well. Mrs. Thomas would likely benefit from a walker or cane, and needs to be educated on the need for assistance when ambulating. The PHN would assess the safety of the environment at the Thomas’ home. As Mrs. Thomas becomes weaker and has more trouble ambulating, they would need to get rid of any area/throw rugs that could make her fall. They would need to install grab bars in the bathrooms, make sure the house was well-lit, and that handicap features were installed where necessary, such as ramps at the front…

She states she was walking when her right leg just “gave out”, and she fell on to the right hip. She complained of mild pain in the right hip, and mild edema was noted in the ER. In addition she had external rotation of the right leg. Initial x-ray demonstrated findings of intertrochanteric fracture nondisplaced of the right hip. Consultation was obtained from Dr. Dodd who concurred with the diagnosis and treatment recommendations were made.
