On Saturday, April 16, 2016 at approximately 10:00 a.m. Resident Assistant (RA) Jessica Estrada, Resident Assistant (RA) Christina Mbadie and Resident Assistant (RA) Freddy Cardenas began their monthly room inspection in room 216. RA Jessica began to knock on room 216B, when she realized that the door was already opened. RA Jessica noticed duct tape on the door strike plate. She captured a picture of the door strike plate before removing the tape.

Once in the room, RA Jessica noted the clothes on the bed, empty cartoons and headphones in the desk, clothes in the closet, books on the floor, and the bag under the bed, since the room was supposed to be vacant. RA Christina then informed RA Jessica that the items belonged to resident Nathaniel

    On Thursday December 31, 2015 at approximately 1949 hours, FHEO Security Officers were dispatched to room #503 Beds 1 and 2 for two Disorderly Male Baker Act Patients who were acting up and causing problems for their assigned Psych Sitter, including one patient from Bed 1 who kept trying to leave the room. Security Officers Omar Alonso, Carlos Ayuso, and James Johnson arrived and met with Nurse Celia Baggya and Psych Sitter Andell Philip while Security Supervisor Steven Evans arrived later and met with Charge Nurse Charlene McGee.
