Reflective Practice
I am writing this reflective journal in accordance with the CNO`s reflective practice standards and the LEARN model. Though my time on 600A has only just begun I have learned so much. Like many second year students, this is my first experience in the hospital. It has proven to be much different than my previous clinical experiences. My time with my first client stands out in my mind as a key learning experience.

I like to believe that I am caring and compassionate with everyone I meet, especially in a professional setting. This particular client was a lady in her early 80s. She was good natured and very easy to get along with. She was cognitively aware and quite bright. As a result of bowel surgery she had an ostomy. This was new to her and I could tell she was unsure about it. My first day with her we did a lot of chatting and sharing. She told me about her husband, children and career as a kindergarten teacher. This helped build a certain level of comfort with me as a caregiver.

I had her to come for walks with me down the hall and once she was up and moving she said she felt much better. I helped her with her first shower post-op. I made her feel more comfortable by promising to stay outside the door while she was in there. When she was finished, I helped her to apply lotion to her dry skin and at her request gave her a good back rub. My shift ended and I said my goodbyes. My client told me she was sad to see me go and was looking forward to my return the next morning.

That night when I went home I did some research into ostomies. Through my textbooks and previous experience I had a solid knowledge of the basic care but just in case I wanted to refresh.

I was excited to return to clinical the next morning. My client had a big smile for me when I walked into her room. Throughout the evening the ostomy nurse had come in, did some assessments and quickly taught my client how to empty and clean the appliance. Unfortunately since the nurse was

    When I introduced myself to patient A for the first time she seemed very distressed. She was lying in bed on her side. She had a notepad and pen in bed beside her. She was able to write down that she felt very uncomfortable in the bed. I quickly learnt that she was also able to give a ‘thumbs up’ gesture for a yes as an answer and a ‘flat hand’ as a no answer.     What is reflective practice? In short, it’s a practice that helps you become an active learner. Many of us, including myself, may be considered or interpreted as passive learners. Passive learning goes a bit like this: we read textbooks, we follow everything our instructor or teacher tells us (sometimes we tune out), we write exams and then…we forget what we learned. It’s common and though not everybody goes through this, historically, it happens more often than not. However, with active learning we engage in conversations with our classmates and teacher, we ask questions, we figure out answers, we contribute and learn and teach each other. And ultimately, we engage with ourselves (and seldom tune out as a result). Oxford dictionary defines reflection as “a serious thought or consideration…an idea about something, especially one that is written down and expressed”. Reflective practice facilitates coping. From a personal perspective it is different for each of us like writing personal journals or talking on the phone with family/friends.     Reflective practice is frequently utilised throughout nursing care. It allows the healthcare professional to explore their past and present experiences, their concepts of current practice and enables the ability of identifying means to enhance ongoing development (Levett-Jones & Bourgeois 2007, p. 117). Mann, Gordon & Macleod (2007, p. 595) highlights that current healthcare professionals must function in complex clinical systems. In order to do so must activate their own awareness to certain needs or interruptions which occur on the provision of nursing care. In purpose of this response I have elected the Gibbs (1988) reflective cycle which incorporates description, feelings, evaluation, analysis , conclusion and action plans to structure my reflection and deconstruct my chosen issue (Bulman 2008, p. 226). Due to the poor instruction given for this discussion l have chosen to structure my reflection to critically explore the significance of establishing a therapeutic relationship in nursing care to holistically improve best practice and patient health outcomes and the consequences amd damaged caused by choosing to ignore the importance of this relationship (Bulman 2008, p. 226; Schutz 2007, p. 27).     The client was a resident of the Davis Child Shelter, her name was Resident S. The interview happened 8/7/15. Most of the residents went outside to play some basketball and football in the afternoon, and she was seeing at the steps alone, and looked distressed. I walked over to her, to see what was wrong. There was no previous interview, and before approaching Resident S, I was not sure about the readiness if she was willing to talk to me.
