Qdp Observation
Within this quarter, QDDP informally observed participant’s progress on a daily basis as the information was gathered for this 90 narrative and communicated by staff to include follow up with staff on participant’s progress on loading/unloading dvd player, washing hands, and pulling up her undergarments and wiping/cleaning the kitchen counter. During observation on Mar 14th, QDDP directly observed Sherri demonstrate her ability to load and unload the dvd player. Unfortunately, this task was not the task of the day. She grab the dvd and threw it on the floor and walked away. Staff waited and tried to redirect her to the dvd player and she refused. On April 12th, QDDP directly observed Sherri, as she demonstrated her ability to pull up her undergarment after using the restroom.

Staff provided hand over hand guidance and asked Sherri to pull up her undergarment. Sherri laughed, and refused to cooperation with the request for her to pull up her undergarment. QDDP intervened and assisted Sherri and she refused. On this same day QDDP, directly observed demonstrated her ability to wash her hands before meals. Staff provided hand over hand guidance and assisted Sherri to turn on water (this task failed-she refused); Staff assisted her in adding soap (Sherri refused this task as well). Staff placed her hand under the soap dispenser, pumped soap into her hand and wet her hand. Staff then, assisted her in lathering and scrub hands while under the water (Sherri successfully performed this task). She rinsed with hand over hand guidance, dried hands with hand over hand
