1. Gather necessary information to apply for the NCLEX exam and license as a licensed practical nurse
a. To apply for NCLEX you must have:
i. Proof of citizenship ii. Transcripts showing degree and date of completion iii. Full set of fingerprints (Fee: $28.75) iv. Photograph of yourself
v. License fee (between $30.75 and $123 depending on month and year of test) vi. Conviction information vii. Any disciplinary actions
b. You must register and pay for NCLEX examination online or by phone
c. Allow 4-6 weeks for fingerprint processing
d. Allow 8-10 weeks receipt of a complete application to process licensure decision.

Must have practiced nursing for at least 500 hours during the past 5 years and completed 20 contact hours of continuing education within past 2 years. iii.
