Posaconazole Case Study
1. Title: Cost-Benefit analysis of voriconazole versus posaconazole for antifungal prophylaxis in patients with acute myeloid leukemia undergoing induction chemotherapy in Australia
2. Research by: Al-Badriyeh D, Slavin M, Liew D, et al. Pharmacoeconomic evaluation of voriconazole versus posaconazole for antifungal prophylaxis in acute myeloid leukaemia. Journal of Antimicrobial Chemotherapy. 2010; 65(5): 1052-61
3. Condensation and Commentary by: Hanna Yahya Mohamed, Final year pharmacy student, Qatar University
4. Condensation of the Research:
Purpose of the study
To evaluate the clinical and economic outcomes of voriconazole and posaconazole when they used as antifungal prophylaxis in acute myeloid leukemia (AML) patients undergoing induction

Since 2002, voriconazole, a triazole antifungal agent, was used in practice as a prophylaxis drug against IFI in neutropenic patients with AML (2,3,8), deposit that there was no large randomized studies performed - at that time- to assess its efficacy in preventing IFI in these specific patients (2,8). On the other hand, posaconazole, which was introduced in the market in 2006, was shown to reduce the incidence of IFI in patients who went through induction chemotherapy for AML (8,9). For example: Cornely et al. showed that posaconazole prophylaxis is superior to fluconazole or itraconazole in reducing IFI incidence (occurrence rate is 2% in posaconazole arm versus 8% in fluconazole/itraconazole group, p-value <0.001) and mortality rate (P=0.04) during AML chemotherapy induction phase (10). However, studies that evaluate the efficacy of posaconazole compared with voriconazole are yet not performed. Thus, there is a gap of knowledge in determining if posaconazole will be also superior to voriconazole (the standard line) in preventing IFI in AML patients. Moreover, there is no pharmacoeconomic study preformed to compare these agents for that …show more content…
provided the first pharmacoeconomic study that investigates the clinical effectiveness and cost associated of voriconazole versus posaconazole as a prophylaxis from IFI in patients with AML induction chemotherapy (3). The purpose of this work was to evaluate the direct healthcare cost and clinical effectiveness of each drug in the primary prevention of IFI in AML patients undergoing chemotherapy (3). Based on the objective and the perspective of the study, cost-benefit analysis was suitable in this study because it allow for comparing multiple outcomes (like discontinuation of the therapy, therapeutic success …etc) in single

With regard the oral thrush infection, the patient had a full history taken and clinical examination. A holistic viewpoint that took into account the fact that the patient was experiencing unpleasant side effects led the nurse to conclude that GP referral was necessary to confirm the diagnosis and also to tackle the concurrent issue of the steroid inhalers and depression. When considering step four, the NP had reviewed the British National Formulary online (BNF, 2010a) and had decided that Fluconazole was the most effective product for this patient as the patient also had a dry mouth because of her oxygen therapy. Nystatin, another antifungal can be used for oral thrush, but it is less suitable for patients who have dry mouths (BNF, 2010b). The patient also had no contra-indications to Fluconazole, and she was not taking and other drugs that had known drug-drug interactions with this new agent. The patient was informed of all the possible side effects in a form that she could understand.…

High costs for chemotherapy and treatments: A weakness in the oncology field is the high costs for purchase and reimbursement of chemotherapy and other treatments. Insurance companies have significantly cut back on the amount they will reimburse for treatments, which in some cases, leaves us not collecting what was paid for the medications. This has become quite the challenge to insure we are being reimbursed enough for the therapies that have been prescribed for our patients by the physicians.…

Separately and coincident with the closing of the Agila transaction, a number of key commercial enhancements have taken place that will reduce Agila's historic, long-term reliance on a business-to-business model and increase value to Mylan. These changes unlock commercial rights and economics in many of Mylan's key growth markets and improve the long-term commercial profile of Agila for Mylan. Some of these key changes include regaining full commercialization rights for most of Agila's U.S. portfolio, both marketed and pipeline products, including all oncolytics, and regaining significant product rights in Canada, Australia, Brazil, Japan and South Kore…

Results of this study found that a single dose of Oritavancin was just as effective as Vancomycin in shrinking the infection, lowering fever, and even curing cases of MRSA. Dr. Corey states in this article that “this new drug could reduce the amount of time patients would spend in the hospital”, I believe that statement to be true. I am employed as a Paramedic in Las Vegas, and in my particular profession we deal with patients seeking treatment for infections almost on a daily basis. Patients are being brought from care homes and other facilities to the hospital regularly for treatments such as Vancomycin infusions. In severe cases those patients end up being hospitalized until the infection is gone.…
