I agree with you that medication reconciliation is an important strategy to negate the adverse effect of polypharmacy in our elderly clients. Equally important, Jyrkka, Enlund, Korhonen, Sulkava, & Hartikainen (2009, p. 502) asserts the idea of defining the effectiveness of the medications on a consistent basis with emphasis on actual effects and outcomes of drugs to reduce the harm to the patient and maximize the efficacy of the treatment. I also believe that physicians have to perform a medication follow up and diagnosis review considering sometimes the patient condition can be misdiagnose resulting to inappropriate medication therapy.
