Personal Narrative-Steroid Injury
DOI: 2/8/2005. The patient is a 47-year-old male general laborer who sustained injury to his lower back and groin area while assembling solar panels in a warehouse.

The patient is diagnosed with other intervertebral disc degeneration, lumbar region; low back pain; and chronic pain syndrome. Per medical report dated 5/6/16 by Steven Manager, M.D., the patient returns with complaints of lower back pain and right leg pain. He described pain as aching. He rates his pain as 3/10. In addition to pain, the patient also complains of joint pain and stiffness and muscle spasms. He is also trying home exercise program, breathing and relaxation, transcutaneous electrical nerve stimulation (TENS) unit, and ice and heat for pain relief. His social activity level has increased. Varav. An evaluation was performed on May 31st by Dr. Varav. Of note, Dr. Varav’s consultation report notes that the patient has history of low back pain with radicular symptoms down the right leg. It also notes that the patient had his work injury in 2005 and has benefited from epidural steroid injections in the past. It also notes that the patient has weakness in the right lower extremity and knees for pain, and his ability to improve both his subjective reporting pf pain, as well as his functional aspects in his ability to perform activities of daily living, with the use of medication, including other medications such as cannabis. He has previously been off of his medications and had conservative management with chiropractic manipulation. But since this was discontinued, he had to resort to medications, and he has gradually weaned or tapered down on his opioid medications, to his current dosage of OxyContin 40 mg twice a day, with Oxycodone 30 mg 3 times a day, and Norco twice a day for breakthrough pain.
