Nursing Leadership and management
Continuous quality improvement (CQI) is the responsibility of all nurses and is vital when addressing the challenges of the health care industry. Provide an example of how you would apply CQI in your current or past position.
The purpose of QI is to use a systematic, data-guided approach to improve processes or outcomes (Conner, 2014). Principles and strategies involved in QI have evolved from organizational philosophies of total quality management and continuous quality improvement.
Connor (2014) emphasizes that while the concept of quality can be subjective, QI in healthcare typically focuses on improving patient outcomes. So the key is to clearly define the outcome that needs to be improved, identify how the outcome will be measured, and develop a plan for implementing an intervention and collecting data before and after the intervention.
Connor (2014) points out QI methods as follows:
Various QI methods are available. A common format uses the acronym FOCUS-PDSA:
Find a process to improve.
Organize an effort to work on improvement.
Clarify current knowledge of the process.
Understand process variation and performance capability. Select changes aimed at performance improvement.
Plan the change; analyze current data and predict the results.
Do it; execute the plan.
Study (analyze) the new data and check the results.
Act; take action to sustain the gains.
Unlike research and EBP, QI typically doesn’t require extensive literature reviews and rigorous critical appraisal. Therefore, nurses may be much more involved in QI projects than EBP or research. Also, QI projects normally are site specific and results aren’t intended to provide generalizable knowledge or best evidence states Conner (2014). Examples of QI projects include implementing a process to remove urinary catheters within a certain time frame, developing a process to improve wound-care documentation, and improving the process for patient education for a specific chronic disease.
The institution I have been at has been working on decreasing central line associated bloodstream infection and peripheral line blood stream infections. Numerous risk factors for CLABSI associated with needleless connectors have been proposed. Some are attributed to poor hand washing before manipulation, inability to properly disinfect the connection site due to poor design, aseptic device management, and frequency of the connector exchange. In addition, several studies looking at intraluminal contamination from the needleless connector have demonstrated that high levels of contamination can be seen colonizing the connector and subsequently moving into the CVC (Ramirez, Lee, & Welch, 2014). Even with varying levels of disinfection, colonization can still occur. Studies of current I.V. practices demonstrated that 56% of Registered Nurses typically do not believe it is necessary to disinfect catheter hubs and >90% of nurses do not cover an intermittent infusion (Ramirez, Lee, & Welch, 2014).
The 70% isopropyl alcohol cap known as “Curos” is used as an attempt to reduce intraluminal contamination. The cap requires ongoing training and encouragement to change practice. Studies indicate that consistent use of the caps does influence CLABSI rates substantially in terms of morbidity and the financial resources that are expended because of CLABSI’s.

Nurses are expected to deliver high quality care with the latest techniques and information available. In order to do this nurses are required to actively participate to nursing research and critique them. Nursing research has increased greatly in the past three decades exposing nurses to latest knowledge, to provide quality care to the patients (Polit & Beck, 2004). Further more, to provide evidence based practice, nurses must be able to assess and critique research to evaluate and to judge whether the research is useful, good quality, current and safe to apply in their practice (Fink,2005). Quality management makes sure that the standard patient care is being met while proving quality patient service. Risk management and infection control are also monitored in quality management to insure the patients receive quality care. Total quality management does not focus on an individual when a problem happens but rather focuses on the system as to why the problem happened. Continuous Quality process improvement focuses on positive process improvements to provide quality care to patients and prevent unnecessary pain.…

The changes necessitated by healthcare reform and consequences of the economy are challenging hospital administrators to decide how they will thrive and respond. A change instituted by the Chief Nursing Officer (CNO) at the hospital where I am employed is the discontinuation of special pay practices. The special pay practices involved are compensation for 40 hours per week when actual worked hours are 36 and compensation for at least a 4 hour minimum. This change has a positive financial impact which allows for growth of the organization. However, if not managed…

In this paper we will discuss the integration of evidence based practice into professional nursing practice. Scott & McSherry (2008) define evidence based practice as the combination of individual, clinical, or professional expertise with the best available external evidence to produce practice that is most likely to lead to positive outcomes for a patient. Despite literature surrounding what evidence based nursing is and isn’t, nurses struggle to get evidence into practice. Many reasons have been reported including a lack of understanding about evidence based nursing means. Scott & McSherry (2008) also define evidence based nursing is a process by which nurses make clinical decisions using the best available research evidence, their clinical expertise and patient outcomes. We will also discuss nursing’s simultaneous reliance on and critique of EBP in the context of critical reasoning. There will also be discussion of a study done that examined the effects of integrating evidence based practice into clinical practicum among RN-BSN students and the limitations of evidence based practice and an alternate view of decision making. Lastly we will discuss evidence that challenges the traditional practice regarding injection sites (Cocoman & Murray 2010).…

Evidence-based practice (EBP) has been emphasized since the publication of the Institute of Medicine’s report “To Err Is Human”. Nurses should be able to not only provide standard care within the scope of practice but also deliver safe care based on the best EBP that is most up-to-date at that time. Furthermore, research is developed based on the current evidence-based practice. Research nurses work on new projects to conceptualize innovative ideas, generate literature research, and conduct clinical trials to develop new evidence-based practice. The reason why evidence-based practice and research are important is because health…

Throughout this process I have experienced many different feelings towards my managers and experienced several different types of management styles as related to our recent management concept learning. The TQM strategy has four components- CARES+, EQUIPS, Quality Improvement Teams (QIT) and Critical Paths. CARES+ is a quality planning and reviewing process. EQUIP is an employee suggestion program for increasing revenue, cutting cost, or simplifying work processes. For productivity improvements, soliciting ideas from workers are important. Quality improvement teams were empowered so that they could construct their own approaches to solve thorny problems like long queues in the emergency room. Critical paths approach was added to TQM strategy for managing…

Can you elaborate?] for every department with specific targets related to quality, service, people, and finances. Attract and Retain the Right People High-quality physicians, nurses, administrators, and ancillary staff are critical to producing high-quality outcomes and effective quality improvement. Top-performing hospitals stressed the need for selective hiring, credentialing, and re-credentialing. Successful recruitment and retention of nursing staff was tied to an absolute respect for and empowerment of nurses—who must be treated as full partners in patient care and given opportunities for advancement. All are expected to be good team players, able to participate in multi-disciplinary teams for both QI and patient care management. Among the hospitals studied, examples of dedication to attracting and retaining the right people include: preservation of nurse-patient ratios even during layoffs and at the expense of revenues; a policy that results in loss of staffing privileges for physicians who do not show respect for nurses; and a QI residency elective to introduce medical residents to QI philosophy and techniques. Nurses are responsible in providing holistic, quality care to their clients. In order to effectively provide such care Boswell and Cannon (2009, p. 2 & 7) states that nurses must base their provision of care on the most current, up-to-date health information available and sound nursing knowledge. This is where evidence-based practice (EBP) comes in. Polit and Beck (2010, p. 4) defined EBP as "the use of the best clinical evidence in making patient care desicions". This usually comes from research conducted by nurses and other healthcare professionals. Thus it is pertinent that research reports are critically analyzed.…
