Mrs. Lecaine Case Summary
Mrs. Lecaine returned to Michigan for the summer. She continued to report low back and abdominal pain. She states when sitting up she has a bulge in her abdomen that is painful. She has just begun to resume physical therapy, was sent for an Ultra Sound of the abdomen. Before the results are know, she has decided to go back to their home in Colorado. Mrs. LeCaine has 6 hours per day of attendant care orders. T he reason for this need was not fully investigated before she left for Colorado.

SUMMARY

On 8/6/15 Mr. and Mrs. Lecaine were restraint driver and passenger in their vehicle. They were at a complete stop when they were rear ended and pushed into the vehicle stopped in front o them. The car that hit them was going approximately

    HISOTRY OF PRESENT ILLNESS: This 40-year-old Latin female presents with complaints of low back and right leg pain she said that she hurt her back in a motor vehicle accident three years ago and she has had a history of intermittent low back pain since that time. Last December she started a job where she had to lift boxes that weighed approximately 40 pounds.
