Moving and Handling
On Saturday the 13th July I entnered the pass code for the building and then fobbed it this is to show administration staff who is all in the building. This complies with the Fire (Scotland) Act 22.01.2001. The I was given our daily morning report of the residents living in the home at the time. On this patricidal day the nurse asked me to assist Mrs M and all aspects of her personal care.
Having had the report I then asked my nurse in charge if I could look at Mrs M care plan to get any information about her. I had to ask my nurse to look at the care plan due to the Data Protection Act 1998 which Mrs M is protected by. After having looked at the care plan I found out her mobility was poor and would need at stand aid which a risk assessment has been put into place for, so Mrs M was able to be transferred. As Mrs M would need a stand aid to help with all transfers myself and another career would have to carry out the task together due to two acts, the moving and handling act 1992 and also the Health And Safety At Work Act 1974. The Health and Safety at work Act was put into place to protect myself and the resident from hurting ourselves. I then notified the nurse in charge that myself and another carer where going to Mrs M room to assist her so they knew where we were. On arrival to Mrs M room I chapped her door and waited for her to reply for yourself to come in as we have to respect her privacy, as the room is Mrs M we have to wait for her permission to enter the room. We get permission and entre where we greet Mrs M with and good morning and introduce ourselves so she was able to name us. I went over to the top of the bed where she was lying and bent down to her level. I spoke to her in a friendly matter as I would speak to someone who has no problems communicating this is part of the SSSC codes of practice. I then ask Mrs M if she would like a hand to be wash and dressed, to Mrs M replied yes she would like assistance to wash and dress. We have to ask this

Last summer whilst I was talking to one of the residents she remarked about how varied the bird-life was that came into the garden and that she would like to find out what they were and encourage more birds into the garden by getting a new bird table. I spoke to the manager and she agreed for me to take the resident to the local garden centre to choose one. I said that I would be happy to take her along to the garden centre on Thursday as it would not leave the shift short staffed. I advised the resident that I had spoken to the manager and that I would pick her up on Thursday morning after breakfast and take her to the garden centre. On the way home I called into the garden centre and arranged with them to park in one of their disabled bays even though I did not have a badge to display as I was bringing an elderly resident to the garden centre and she could not walk great distances. They agreed that this would be no problem.…

Offering inappropriate treatment or care is unacceptable within a care setting because every resident deserves to be treated equally and professionally. As a care worker it is your duty to ensure that they are receiving the best care possible no matter why they are in the situation they are in. A resident’s past should not affect the way you treat or think of them, we are there to support and promote independence for all residents. If a resident is showing unusual behaviour you should try to find out what is wrong with that resident and don’t label them as being awkward or doing it for attention.…

However, my client has the ability to take actions by telling my client’s mother on what happened with a few words, sad emotion (crying) and demonstrating what happened with his hands. Although my client can’t speak or talk effectively, the carer can set up a few cameras that will record everything at each corner of the building my client is been taken care of by the carer; my client’s carer can see what’s happening when my client is been taken care of by the carer; my client’s carer can tell the police and charge the carer with the recorded video footage of the camera to the police. Furthermore, my client’s carer can give the carer a warning or fire them from their job, which is unlikely, as the client has violated the Code of Practise; the carer should be charged (like they can’t work as a care assistant), as they shouldn’t be taking care of other…

I was just starting out working as getting my clinical hours and I remember looking at my patients care plan and I didn’t see anything about this particular patient being a DNR = Do Not Resuscitate and I went into the patients room to check on this man and I realize that he isn’t breathing and I called the nurse as quick as I could and I went into the room and was about to start chest compression and as I was about to start I heard the nurse shouting at me don’t touch him. The nurse didn’t do anything as well I was upset because I was under the impression that we suppose to do everything in our power to keep these patients alive. Me and the nurse argued because I didn’t see the DNR on the man’s chart and as I sat down with the charge nurse she showed me where it was located.…

I felt empathy for the service user but stated that all moving to and from the bed or chair had to be carried out using the hoist which is stated on the risk assessment for her safety and it is against the moving and handling policy not adhering to the care plan. The service user was further advised that since she had refused to allow the hoist to be used, unfortunately she had to stay in bed or be lifted by a member of the family. It was my responsibility to have adhered to the moving and handling policy for my own safety and that of the service user whose mobility is impaired.…

My preceptor had explained to me the process involved in care planning for a patient on the unit, the doctor will do the majority of the assessment, the nurse carries out the risk assessment and completes Roper Logan and Tierney nursing assessment which is the nursing model used by the Louth/Meath services. The nurse also carries out an admission checklist. When the patient has been admitted and the nurse has gathered all the relevant information they will then incorporate the care plan. I familiarised myself with the documents, I will admit I felt a little apprehensive; I was worried I might say the wrong thing or ask the wrong question. As John had a previous history, I had been informed that he suffers severe paranoid delusions. This immediately alarmed me in the sense of communication difficulties.…

Mary (changed name) was admitted with right cerebrovascular accident (CVA) and was now just waiting for residential placement. One of the side effects left over from her stroke was that she had a drop-foot for which she had a splint and a walking stick; she had declined to use a frame. Mary was one of the patients on my team so I was getting to know her quite well during my first two weeks on the ward. I was working with another patient when I saw Mary twist on her ankle and try to steady herself on her stick. This had the effect of spinning her round and she fell to the floor onto her left hip. I ran to help, she was in a lot of pain and slight shock, I called down the ward for help several members of staff came to help saying "do this" and "do that" all at the same time, I was quite over whelmed. Mary was rolled back and forth onto a sling, hoisted up and placed on her bed, all the while I was looking after her head, I did not know what else to do so I talked to her to calm her down, which was difficult due to all the commotion. On initial examination the senior nurse on duty said it looked like she had broken her femur, which was later confirmed. Her residential placement was cancelled and she was transferred to stepping hill.…

Kinematics is the geometry of pure motion - motion considered abstractly, without reference to force or mass. Engineers use kinematics in machine design. Although hidden in much of modern technology, kinematic mechanisms are important components of many technologies such as robots, automobiles, aircraft, satellites, and consumer electronics, as well as biomechanical prostheses. In physics, kinematics is part of the teaching of basic ideas of dynamics; in mathematics, it is a fundamental part of geometric thinking and concepts of motion. The development of high-speed computers and robotics, and the growth of design synthesis theory and mechatronics have recently revived interest in kinematics and early work in machine design.…

I have learnt to be tolerant and to understand other people’s cultures, to respect beliefs and values of other people. This excited me, and I felt I wanted to find out how much I had learnt from this module and what to apply to my placement. I wanted to find out if the patients had individual rights at these homes. I was keen on observing how the staff cared for the aged, and to see if the standard set by the Care Homes Regulatory Office were being met. I was surprised to find that the department of health has stipulated the National Minimum Standard for the Care homes for Older People. I also felt apprehensive about being excluded by the carers, on the groups of respecting the patient’s private needs, if any. Nevertheless I was also concerned about the new environment of working in hand with the carers who understood what was expected from them. This was a challenge for me I then confirmed my…

Therefore, I ask Richard if the patient’s skin had any breakdown, he reported the skin was intact. Consequently, during my morning assessment, I asked a team member to help me reposition the patient. Hence, when I turned the patient, I found her on a bedpan. Moreover, I apologized to her, because I had not been told she was on the bedpan. The patient replied, “I have been on the phone since the day shift yesterday, also I have asked three people to get me off, no one came back.” Consequently, when Richard came back, I confronted him with my findings, he claims he did not put the patient on the bedpan all shifts. I explained to him, the patient’s injuries were severe and he had reported no skin breakdown, also charted the same, therefore, I informed him I must file an incident report. Suddenly, he became quite and went into the manager’s office to defend himself. Then, during the investigation, the previous day shift nurse of the patient, claimed she had put the patient on the bedpan, before leaving and had informed Richard. Consequently, a few days later Richard was…
