The purpose of this project was to gain experience in quality improvement by working with an organization to identify a business problem, analyze the causes, develop an improvement plan, implement changes, and to verify the effectiveness of the solutions. For this project we had to keep scope in mind. This project had to follow certain criteria, such as: that it is a manageable project with a local organization, it can be completed within the time frame of the course, it has a direct impact on external customers, it is relatively simple, and that it is not something that is currently undergoing major changes.…

Recent efforts have led to an agreed upon unit of measure for application improvements (requirements as opposed to projects, sub-projects or tickets). Within the past year, we have been able to tie the systems of records for projects, testing and production performance together, but the quality of consistent data is still lacking. We have only been able to create a few localized reports that show cause and effect. For example, 4 or more change requests within a single project will lead to an exponential number of testing defects.…

The second core competency that is important to me is Quality Improvement. A responsibility of my position as an application analyst is to build order sets within the electronic medical record. I belong to a multi-disciplinary team of pharmacists, nurses, administrators and providers that are continually analyzing medication order sets for improvement in patient outcomes. Our committee uses a Quality improvement model to make enhancements to our order sets using The Plan-Do-Check-Act. “The Plan-Do-Check-Act (PDCA) cycle is shorthand for testing a change –by planning it, trying it, observing the results, and acting on what is learned” (The Massachusetts Nursing Core Competencies: pg. 20, para 5). The committee discusses a change that needs to be made to the order set, the change is implemented, and data collect on the change, if the outcome of the change is positive no action is required. However, if the change is not what was expected, it is taken back to the committee.…

Various methodologies exist for the integration of quality improvement strategies into performance improvement measures. With concepts of total quality management (TQM) and quality improvement (QI) being introduced to health care organizations; administrators have had to decide which methodology is right for the organization. There are numerous methodologies: Six Sigma, Lean, and Customer Inspired Quality. Each has its own pros and cons. A key component of quality improvement is the technology that gathers and compares the data that the quality improvement measure produces. All of the information that is gathered from the technology can be benchmarked against other health care organizations.…
