The health care organization I am affiliated with, Staten Island University hospital, is part of the North Shore –LIJ Health System. “The North Shore-LIJ Health System strives to improve the health of the communities it serves and is committed to providing the highest quality clinical care; educating the current and future generations of health care professionals; searching for new advances in medicine through the conduct of bio-medical research; promoting health education; and caring for the entire community regardless of the ability to pay” (North Shore LIJ health care, 2014). In 2007, as part of their commitment to quality clinical care, North Shore LIJ began implementing health information technology and electronic medical records in the hospital system. “ The North Shore-LIJ Health System announced today it is subsidizing up to 85 percent of the cost of implementing and operating an Electronic Health Records (EHR) system in the offices of its more than 7,000 affiliated physicians in New York City and Long Island -- part of a…
The pros with the acquisition is that the steering committee worked well together and brought in someone from the outside that had knowledge of EHR. When the process was one and presented to the staff they were willing to work with the new system. The new system will provide health maintenance reminders, drug interactions, and access to clinical practice guidelines or standards of care. The practice also intends to take advantage of the CMS EHR Incentive Program and achieve meaningful use. The implementation of EHR system will enable Valley Practice to be recognize as a high-tech, high touch practice that…
The Health Information Technology for Economic and Clinical Health Act (HITECH) is a part of as part of the American Recovery and Reinvestment Act of 2009 (ARRA). ARRA contains specific incentives that are designed to speed up the adoption of electronic health record systems. According to Rouse ( 2014), “HITECH stipulated that, beginning in 2011, healthcare providers would be offered financial incentives for demonstrating "meaningful use" of EHRs until 2015, after which time penalties may be levied for failing to demonstrate such use. ”HITECH and HIPAA, are different and they have unrelated laws, but they do meet in some laws that shares the same goals. For instance, HITECH has Notification of data Breach rules and requirements for unauthorized…
This is a second Medicare compliance determination. The initial compliance determination was made by EHR which felt the patient should be observation status. Dr. Neilian disagreed. My clinical review of the chart reveals that the patient is 89-years of age, she presented with shaking, chills, fever of 103 and hematuria. Her medical history is significant for a previous history of right breast cancer, chronic acquired lymphedema, as well as hypertension. Her initial labs showed her to have an elevated white blood count and left shift, as well as a positive blood culture growing E coli. I believe that this patient represents acute inpatient admission. She requires IV antibiotics so I concur with the attending physician Dr. Neilan who believes…
Introduction This is a case study about an older woman (Gladys) with dementia but has mental capacity. She receives care and assistance in her home, she has a cleaner who comes in to clean once a week, a neighbour who makes her dinner (Gladys pays for it) and a granddaughter who is her advocate. She has mobility difficulties but can walk short distances with a trolley/Zimmer frame. She spent 5 weeks in the hospital receiving treatment for a urinary tract infection (UTI) and was appointed a care manager after her discharge. Gladys has over £25,000 and receives £600 as pensions monthly.…
EHR can be a benefit to providers and patients because it allows providers to access patient's medical records effectively. This is good for several reasons. Test results and other important information can be available at the time and place that a physician needs, allowing them to provide better care to their patients. It also prevents tests from being repeated and eliminates a lot of mistakes that can be caused by using paper filing systems. EHRs also allow providers from multiple areas to collaborate, and to share medical records in order to care for patients more effectively. Preauthorization can eliminate problems with insurance eligibility, avoiding delays in payment for doctors and unnecessary costs for patients. Patients also receive…
Hospitals, doctor’s offices, and care givers must meet the criteria of Electronic Health Record Program to become eligible for the Medicaid and Medicare Services (CMS). The first year, hospitals and Eligible Professionals (EP) do not have to demonstrate their important use, but are obligated to approve or upgrade to an (EHR) for them to receive payments for the services rendered. All work must be documented correctly in stage 1, (data capture & sharing)- stage 2 (Advance clinical processes), or stage 3 (Improved Outcomes)…
On July 13, 2010 the Centers for Medicare and Medicaid Services (CMS) as well as the Office of the National Coordinator for Health Information Technology announced final data standard rules for hospitals and physician offices to implement a phase in process beginning in 2011 to qualify for financial incentives paid to them by CMS. This initiative was detailed in the Health Information Technology for Economic and Clinical Health (HITECH) Act, enacted as part of the American Recovery and Reinvestment Act of 2009. The legislation calls for EMR systems to be certified by CMS to provide “meaningful data” with regard to their overall electronic records systems. This brief is prepared to inform all enterprise stakeholders, including company shareholders, the HCA Physician Services Executive Management team, and practice level managers and physician staff regarding the regulation and its impact on practice operations.…
Bush established the Office of the National Coordinator of Health Information Technology in the Department of Health and Human Services to develop standards to varying EHRs applications. This initiative was made to make EHRs available to Americans by 2014 ("Electronic Health Records: History in The Making", 2013). The benefit of the EHRs is having patient information centralized and easily accessible. This will improve quality of care. EHRs will help providers reduce medical and prescribing errors by improving the accuracy and clarity of medication records and prescriptions. EHRs can increase efficiency by reducing redundant tests and services as well as delay in treatment. Thus, decreasing the cost of healthcare and increase patient…

These forms of enrollment have been a major transformation from the previous processes to enroll for Medicaid. The Modified Adjusted Gross Income (MAGI) is the process that states will need to shift to maintain a uniform income eligibility. A “no wrong door” enrollment policy will help eliminate a person’s entry either through a Medicaid agency or an exchange. Even if a state chooses not to expand their Medicaid program they must meet the…
According to the Lutheran Family Services website (2017) there are eleven steps to the adoption process, each which are part of the planned change process. The eleven things are: meet with an LFS counselor, fill out an application, set up interviews, conduct and complete the home study, participate in adoption classes, work with LFS staff to complete a family book, wait in the adoptive pool, get matched with a birth mom (and her family), take the baby home after it is born, receive regular support and provide reports to the court, finalize the adoption after six months.…

The electronic health records fits seamlessly with a central cost-saving of health care reform: to shift U.S. health care from an expensive, pay-per-service system based on quantity to one that emphasizes quality. The goal now is to have medical payments reward good care -- in a way that's difficult to do with paper records.…
