Medical Case Study Of Mr. Malone's Case
Mr. Malone reports he is the General Manager of TA trucking and is working full time.

DIAGNOSIS/DIAGNOSTIC STUDIES/ SURGICAL INTERVENTIONS

None provided.

CURRENT STATUS

On 6/6/17 I met Mr. Malone at the office of Dr. Sirvasta. He arrived to the appointment alone and was not suing a cane to walk. An x-ray was taken prior to meeting with Dr. Sirvasta. The right inner aspect of the ankle has approximately 6 inches of sutures present. Those were removed and steri-strips were placed over the incision line.

CURRENT TREATMENT PLAN

Dr. Sirvasta reported that the x-ray showed there was no cartilage left in the right ankle and he has a damages ankle joint. He said that Mr. Malone is going to need a fusion. I asked what the current diagnosis

    After satisfactory level of general anesthesia was reached and patient was in the supine position, he was further placed in a beach chair position. A longitudinal incision was created over the region of the left AC joint. At this time, sharp dissection was conducted down to the fascial plane. The fascial plane was then further incised, reflecting both the deltoid and the trapezial fascia and the distal aspect of the clavicle undermining the clavicle; at this time we simply proceeded excising the distal 1 cm of the clavicle with use of a reciprocal saw. With completion of this element of the procedure, the margins of the bone were otherwise unremarkable in gross appearance. It was also significant to note at this time the acromial end of the articulation was unremarkable. The wound was irrigated, followed by controlling of punctate bleeding with use of electrocautery, followed by the closure of the deltotrapezial fascia. At this time I further imbricated sutures for stable repair, followed by repair of subcutaneous and dermal planes. A simple dressing was applied. The patient tolerated the procedure well and was transported to the recovery room in a stable manner.…

    Deep to this area of apparent ulceration there is bright T2-signal present at the dorsalateral aspect of the foot at the level of the mid to distal metatarsals suggesting edema and and/or inflammation in the dorsal soft tissues in that region. I do not see definite replacement of the normal bright marrow signal on the T1-sequence within the tarsals or metatarsals. Therefore, I do not see definite osteomyelitis.
