Medical And Therapy Team: Case Study
Circumstance: Liam will maintain schedule appointments with medical and therapy team. Ms. Smalls (MHP) made face to face contact with Mrs. Wigfall (MHS) and Liam.
Action: Liam was sitting in the highchair eating breakfast by himself. MHP notice Liam is getting better eating by himself. Liam is says words and repeating after others. MHP and MHS discuss Liam’s level of energy during the day. MHP and MHS review paperwork and possible needed services. MHP and MHS review medication paperwork and Easter Seals visit summaries.
Plan: MHP will maintain scheduled home visit to ensure Liam’s well-being.

    The features of effective team performance are set out in Brian Tuckman’s phases of team development theory, ‘Forming, Storming, Norming, Performing.’ This outlines the phases that a team will go through in order to become effective and reach maturity. An effective team will always consist of certain features enabling effective team performance. Some of these features are as follows; develop goals and plans, i.e. an effective team will always devise a development or goal plan, which will describe the specific purpose for the team. The team will work towards implementing plans to achieve the set goal. Team member should also be clear about these goals and their priorities and consistently work in ways that will support the team’s goals. Team meetings should be held on a regular basis, you could also provide a newsletter with such information. These relationships can be enhanced by proactively working to build these positive relationships by providing opportunities for social interaction.
