Md Navigate Case Study
I am just trying to full understand the full workflow of MD Navigate with the Senior Center. How does the transition of care document get sent? Does the physician choose MD Navigate to coordinate, then that will populate the To Location/Pos, and then at that point once the referral is created does the TOC automatically get sent out or should the coordinator need to do something else to effectively send out the summary of care
