Nightingale Community Hospital (NCH) is committed to upholding the core values of safety, accountability, teamwork, and community. In preparation for the upcoming readiness audit, NCH will be launching a corrective action plan in direct response to the recent findings in the tracer patient. Background information on the tracer patient is as follows: 67 year old female postoperative patient recovering from a planned laparoscopic hysterectomy turned open due to complications. Patient developed infection that formed an abscess and was readmitted to the hospital for surgical abscess removal and central line placement for long term IV antibiotics. The tracer methodology was employed when auditors reviewed this patient’s course. Many things were done well and right with this patient and NCH is pleased to know that the majority of items analyzed with this patient proved that NCH was in compliance with regulatory standards; however, there were some troublesome areas that we need to focus on. The primary focus area that we will put our energies into will be the fact that there was not a history and physical completed on the patient within 24 hours of admission, and in fact it was greater than 72 hours before one was completed. The Joint Commission mandates standards that are to be met in order to maintain compliance. Standard PC.01.02.03 states that history and physicals must be documented and placed in the patient’s medical record within 24 hours of admission and prior to procedures involving conscious sedation or anesthesia. History and physicals are also considered in compliance if documented 30 days prior to procedures as long as there are no changes documented or the changes in status are specifically noted. In summary, A Orange County 52 year old woman, Connie Barraclough, was arrested in Newport Beach. She exclaimed that the Police Department violated her women’s civil rights. The woman was reportedly booked in a jail where men are processed to be held in jail. Connie was recently drinking earlier that night at an Angels game in Anaheim, the pursuer who arrested Connie reported that she was driving under the influence and arrested her. This all started when Connie realise she was not processed in the correct place for holding. She was actually sent to Orange County Jail in Santa Ana, because of a mishap of not having a female jailer to be “available” to process her. Rosa Lee Cunningham is a 52 year old African-American single mother of eight, who is a long time heroin addict, and has an extensive criminal record. Of her eight children six of them have followed her in her life of addiction and crime. These crimes include everything from petty theft, to prostitution, to drug trafficking. Rosa Lee has recently showed up at the Howard University Hospital emergency room because her body is trying to shut down due to her heroin addiction, and she has Acquired Immune Deficiency Syndrome (AIDS). The staff in the emergency room reports that Rosa Lee is no stranger to the emergency room, and her records show an extensive 13 year record of drug abuse including heroin, cocaine, and amphetamines. Rosa Lee was referred for counseling for study, in an attempt to get a better understanding of her addictive cycle and an attempt to help Rosa Lee control her addictive behaviors.…

The strengths perspective shows how she is a strong woman who works hard to protect the people in her life. This patient was released form the psychiatric hospital. On a Friday evening in January at the Regional Hospital in Atlanta, Na Yong, refused to sign the release paper. Confidentiality and privacy are words used interchangeably in the medical world when they have very different meanings. Confidentiality is in line with protection of patient information from unauthorized users and privacy is in line with protection of the patient’s physical body from unauthorized users. I personally feel that for me to maintain a high quality of life I need to be able to utilize most of my natural senses. Communication is on the top of my list. I am a verbal personality and I spend a majority of my day talking to people placing emphasis on being able to talk to people and hold an intelligent conversation of substance with my peers and children is vital to me enjoying my life. Each day patients put their physical health and trust in the hands of health care providers. Unfortunately, there have been times when the treatment provided, whether accidental or intentional, has caused harm to the patient. Patients who have experienced injury have the right to lodge a complaint against that provider. Included in the possible reasons for civil complaints is the sharing of personal information, negligence, or assault. These injuries are considered civil wrongs and are covered under Tort Law. In recent years, with the advances in technology, patient privacy has become imperative. In order to protect patient privacy the Health Insurance Portability and Accountability Act of 1996 was enacted, and is part of the Department of Health and Human Services, regulated by the Office of Civil Rights. Health care providers must take care to protect the privacy of their patients at all times. Mary Barnett, the mother of a six month old daughter, left for San Francisco to see her fiancé. Leaving her daughter behind, she returned seven days later to find her child dead.
