3. Hydraulic leak in Blowout
Preventer (BOP) Accumulator identified during a pressure test prior to drilling out the shoe of the surface casing.
The main hydraulic hose connected to the Annular Preventer is leaking noticeably where it is connected to the
BOP.
This could lead to the annular preventer failing during critical use when taking a kick and could lead to the loss of control of well during the kick that could lead to multiple fatal casualties, an acute and chronic impact upon the environment and a significant loss of assets.

Immediate
Do not drill out the shoe to open hole and suspend drilling operations until leaking hose is replaced. Also, ensure the hydraulic fluid level in the accumulator filled to the proper operating level. Midterm
Inspect all hydraulic hoses to all the other preventers to this rig and every other rig in the fleet and function test. After the testing, inspect the hydraulic fluid level.
Long-term
Devise and implement daily inspection checklist of the complete BOP system in BOP connection. Ensure that replacement connections are acquired for all the rigs and kept on each site.
Implement annual inspections of the BOP by properly certified technicians from the manufacture. Today at time of inspection 4. Faulty shear pin in mud pump high-pressure release valve.
The shear pin on the release valve must be of the proper gauge steel that will shear at a pressure lower than the maximum designed pressure of the mud pump and associate highpressure lines. If it fails the pump or lines on the discharge side of the pump could explode causing serious or fatal injuries to workers as well as loss of control of the well leading to a blowout that could have serious environmental consequences.
