UNINSURED MOTORIST COVERAGE (UM) SELECTION/REJECTION
Includes Underinsured Motorist Coverage (UIM)

YOU ARE ELECTING NOT TO PURCHASE CERTAIN VALUABLE
COVERAGE WHICH PROTECTS YOU AND YOUR FAMILY OR
YOU ARE PURCHASING UNINSURED MOTORIST LIMITS LESS
THAN YOUR BODILY INJURY LIABILITY LIMITS WHEN YOU SIGN
THIS FORM. PLEASE READ CAREFULLY.
Uninsured Motorist (UM) coverage provides for payment of certain benefits for damages (bodily injury or death) caused by owners or operators of uninsured motor vehicles. Such benefits may include payments for certain medical expenses, lost wages, and pain and suffering, and are subject to limitations and conditions contained in the policy. (For the purpose of this coverage, an uninsured motor vehicle may include a motor vehicle which has Bodily Injury limits less than your damages. This is
Underinsured Motorist (UIM) coverage.)
Due to Florida Law, all personal auto policies are issued with stacked UM coverage unless non-stacked is selected. If you have previously signed and returned this form, your policy will continue with the same coverages and limits that you currently have. If you have changed your Bodily Injury Liability limits, we must match your UM limits to your Bodily Injury Liability limits until you make another selection on this form. If you wish to make any changes to your policy, please complete, sign and return this form.
At a reduced rate, you have the option to purchase the non-stacked (limited) type of
UM coverage. Under this option, if an injury occurs in a vehicle owned or leased by you (or any family member who resides with you) only the limits of coverage (if any) which apply to that vehicle will be used. (Note that there is no coverage under this policy for a person who is injured while occupying a motor vehicle or motorcycle owned by you or a family member who resides with you which is not covered under the liability coverages section of this policy.)

Under the insured’s businessowners policy, State Farm has a duty to defend and indemnify the insured against any “suit” seeking “property damage” to which “this insurance applies.” Accordingly, in order to fall within the coverage provisions of the policy, the following elements must be satisfied: 1) the person seeking coverage must be an insured; 2) the insured must seek a defense against a suit; 3) the suit must allege “bodily injury,” “property damage,” or “personal and advertising injury”; and 4) the insurance must apply. In the liability section of the policy there would appear to be little dispute that Kathryn Norman is an insured.…
