Manage patient adherence to prescribed regimens of one or multiple drug therapies. Medicare’s private health plan and prescription drug plan members are advised to follow a regimen of medicate adherence to cover 80% or more of the prescribed therapies in a calendar year. The Stars Clinical Pharmacist will target barriers that keep members from adhering to medication regimens. Several medication adherence interventions run simultaneously throughout the year. The Stars Clinical Pharmacist will work closely with physicians to determine appropriate interventions and next steps. Initial telephonic outreach to patients will also be needed in order to optimize medication use, and offer tailored solutions, such as reminder calls, regimen simplification,…
The problems observed are the lack of training and personal care given to the individual patient. Most of the patients are elderly or unable to do for themselves. So we have to take the extra step to make them feel needed and want to give that extra push to live. Also untrained personnel can cause someone to lose their life. Even though this is a small upcoming business all precautions should be taken to ensure safety first. This is a very important aspect for me, because I don’t want anyone hurt from a lack of knowing.…

‘Adherence’ is ‘the extent to which the patient’s behavior matches agreed recommendations from the prescriber’ (1) .We prefer it to the term ‘compliance’ because it includes, but does not presume, the possibility of patient involvement in the treatment decision making process. We do not use the term ‘concordance’ because it assumes a shared decision making process between patients and doctors (2) . Although shared decision making would ideally occur in all clinical encounters, this cannot be…

Medication non-adherence is a serious public health issue, leading to increased health care costs, morbidity and mortality among conditions. Medication non-adherence is also one of the main obstacles for effective hypertension care, as between 20%-50% of the patients do not follow their prescribed medication regimen. Despite the large body of empirical evidence available, relatively little is known about the potential differences between the determinants underpinning intentional versus unintentional non-adherence. Shedding light on the factors of medication non-adherence is crucial to develop interventions and health campaigns addressing the issue. Thus, the present study quantitatively examines the patient-related factors of medication non-adherence…
