Failure Modes And Effects Analysis Model
Step-by-step approach in Failure Modes and Effects Analysis (FMEA), beginning at the earliest conceptual stages of design and continues throughout the life of the product or service, it identifies all possible failures in a design, a manufacturing or assembly process, or a product or service. Any errors or defects that affect the customer are “Failure modes”, that can be potential or actual. “Effects analysis” are failures prioritized according to how serious their consequences are, how frequently they occur and how easily they can be detected. To take actions to eliminate or reduce failures, starting with the highest-priority ones is the purpose of the FMEA. A tool to assist the engineer improve the quality and reliability of a design is FMEA, to include these benefits:

Effects Analysis: The consequences of the various failure modes are determined by their severity to the customer being studied (Goetsch & Davis, 2013).
3. Failure Effects: The failure’s consequence in terms of operation, function, or status of the item (Goetsch & Davis, 2013). A “failure effect” is the result of a failure mode on the product or system function as perceived by the user. Failure effects can be described in terms of what the end user may see or experience (Siemens Product Lifecycle Management Software Inc., 2016).
4. Failure Mode Analysis: An analytical technique used to evaluate failure modes with the intent to eliminate the failure mode in future operations (Goetsch & Davis, 2013). A failure mode is defined as the manner in which a component, subsystem, system, process, etc. could potentially fail to meet the design intent.
