DMHP In Training: Case Study
D: This DMHP in training was called to Mt Carmel by Jodie Mills RN who reported a 55 yr old female, Lynne Guhlke, was brought into the emergency room due to concerns of danger to self. This DMHP traveled to Mt Carmel and spoke to Dr. Amy Theresa who reported Lynne was brought into the emergency room because Lynne’s neighbor called an ambulance due to her behavior and concerns Lynne was going to hurt herself. The neighbor according to Dr. Theresa called several times to the emergency department asking about Lynne’s condition. Lynne’s neighbor, Vivien Leigh, reported to Dr. Theresa that Lynne “gave away her dog” and has been isolating “more and more”.

    Lynn had a unrestful night and into the morning of 7/15/15. The morning medication was adminstered at 7:00 pm. Although Lynn had been given her morning medication and had eaten breakfast, she began requesting and demanding her 1:00 pm medication.     RA would not talk with Treva. RA constantly asked Treva to leave. RA’s daughter & family kept talking to the RA about getting services in the home, especially wound care. Family provided information that the RA will have a physician to come to the home to treat the RA’s bedsores, a nurse to monitor the RA’s meds, and an aide for the RA’s personal care. In report #17-067-00223 the RA left AMA from Hidden Lake Nursing Home. The concern was that his blood sugar was extremely elevated, and he left without taking his oxygen. Treva made a home visit to interview the RA. RA would only talk for a few minutes, but he kept telling Treva that he was fine, and he did not need any assistance from DHSS. Treva was able to explain to the RA that the concern was that he would go into a diabetic coma, but the RA ended the conversation by closing the door. Once the Tarasoft case was opened up to the courts the Confidentiality between patient and psychologist was over depending on the conversation. If a patient like Poddar comes in with psychological issues a psychotherapist should always take action. The Tarasoft decision is a California decision that imposes a duty on a therapist to warn the appropriate people or person when he/she becomes aware their patient may present a risk of harm to a person or persons. The decision made by the psychologist was not a good one at all because the women were in danger. The psychologist alerted campus police because he took what his patient said serious but did not inform her or the family. The Psychologist suggested Poddar should be hospitalized due to his mental state of mind. The psychologist should have breached the patient confidentiality once he knew third party was unaware of the patient conversation with him. Poddar was let go because he seemed to be mentally stable.     It was seemingly a normal quiet night on the day of March 12th 2011, until Virginia Rasnick attempted to phone her husband several times with no response. Sidney Rasnick, while on a work assignment in Jesup Georgia, checked into a motel on March 6th 2011. The hotel was owned by Krishna Hospitality INC.     EREPORT # 19106 stated the following: Leigh Ann Mathis has 4 Children she has lost custody of all of them. She is not allowed contact with Preston. I know this because the Nurses at the Hospital told her to leave or they would call Police. She is now living with Kyle and Preston. She was seen with Kyle and Preston at Christmas.
