CRITIQUE OF PRIMARY ARTICLE B. Purpose of this study The purpose of this study was to obtain information about the measurement of trace metals (zinc, magnesium, copper, manganese, selenium, iron, nitric oxide) and compare it to the severity of sickling of hemoglobin F. C. How was the research/experiment done? What essential values were measured? There were ninety four people chosen for the study. Fifty- nine of the patients had no episodes 3 months prior to the study. The other thirty-five HbAA subjects were blood donors and staff member of the University College hospital. A blood sample of 10mL was taken from each subject. The blood was split up by putting 4mL into ethylene diamine tetra-acetic acid to obtain haemoglobin electrophoresis and estimation of HbF and 6 mL was put into lithium heparin to determine the nitric acid and trace metals. D. What was the conclusion of the research/experiment done? The conclusion is that antioxidant supplement treatment may become helpful in sickle cell disease patients but further research must be done to clarify the observations.

E. Commentary The article relates to physiology because it is researching the different levels of trace metals in the blood. The research shows that and antioxidant supplement therapy could possibly help treat the sickle cell disease. This study was based on Nigerian sickle cell disease patients; I feel there should be other studies done on a wider variety of patients with different ethnicity’s to see if the results are similar to these. Sickle cell is very common in Africa, Central America, Panama, and South America these could be other location that the study can be done. After reading the article I feel the research is off to a good start and if additional research is done and the results of this article are duplicated this can be the beginning for a treatment for sickle cell disease.

Sickle cell anemia occurs when a person inherits two sickle cell gene, one from each parent, that cause the red blood cells to change and become crescent shaped. The underlying problem involves hemoglobin, a component of the red blood cells. Hemoglobin is a protein molecule in red blood cells that carries oxygen from the lungs to the body’s tissues and returns carbon dioxide from the tissues to the lung. In sickle cell anemia, the hemoglobin is flawed (The New York Times). As a result, the cells become sickle shaped and can’t travel as easily through blood vessels. In order for sickle cell anemia to occur is when a sickle cell gene have, been inherited from both the mother and the father, so that the child has two sickle cell gene.     In this lab five blood tests will be performed: hematocrit determination, erythrocyte sedimentation rate, hemoglobin determination, blood typing, and total cholesterol determination. A “hematocrit refers to the percentage of red blood cells (RBC’s) in a sample of whole blood. Since the function of red blood cells is the transport of oxygen to the cells of the body, the higher the hematocrit, the more red blood cells are available to carry oxygen.” In a normal, healthy male the value for males is 47% and in a normal, healthy female it is 42%. Polycythemia refers to a significant increase in red blood cells.” Some causes for this increase in red blood cells are tumors in bone marrow, vigorous athletic training, and living in a place that is higher in altitude. To determine a hematocrit value a whole blood sample is put into microcapillary tube and spun in a special microhematocrit centrifuge. By doing this the blood sample will separate the blood cells from the blood plasma and there will be a “buffy coat” of white blood cells between the layer of blood plasma and red blood cell layer. Then the hematocrit value can be determined “by measuring the height of the layer of red cells in millimeters and dividing that number by the height of the initial column of blood to obtain the percentage of red blood cells.     Sickle cell anemia is the most common form of sickle cell disease which is an inherited, autosomal recessive disorder that causes an abnormal hemoglobin cell.     Hello Mr. and Mrs. Jones. I would like to let you know that if you do have a baby, you are risking him or her having Sickle Cell Disease. What this is is it’s a disease that is genetically inherited and it involves the red blood cells. Mutations in the HBB gene cause this disease. This disease is inherited in an autosomal recessive pattern, which means both copies of the gene in each cell will have mutations. The parents of offspring with an autosomal recessive gene each carry one copy of the mutated gene. The blood cells form an abnormal crescent shape (normal cells are in a disk form). The fragile, sickle-shaped cells deliver less oxygen to the body's tissues. They can also get stuck more easily in small blood vessels, and break into pieces that interrupt healthy blood flow. If in fact your baby does have the disease, you won’t see symptoms until around month four of its life. Symptoms include, abdominal pain, bone pain, breathlessness, delayed growth and puberty, fatigue, fever, paleness, rapid heart rate, and yellowing of the eyes and skin. If you would like, we can do some tests once your baby is born to diagnose and monitor him or her. The three common tests we do are CBC (Complete Blood Count), Hemoglobin Electrophoresis, and Sickle Cell Test. In CBC, we can count the number of red and white blood cells, as well as the amount of hemoglobin in the blood. We would need to draw two samples of blood. In your case, a sharp tool called a lancet would need to be used because your child would be young. The Hemoglobin Electrophoresis also measures the different types of oxygen-carrying hemoglobin in the blood. We would use the same technique as the test prior. The last one, (and the one I would recommend), is the Sickle Cell Test. It determines if there is abnormal hemoglobin in the blood stream that would cause Sickle Cell Disease and Sickle Cell Trait.
