1.3.c Risperidone vs Haloperidol:
Evidence proves that risperidone is often more effective in the treatment of psychosis than the most commonly prescribed typical psychotic medication, haloperidol, which is included in the current list of WHO essential medications with chlorpromazine and fluphenazine. In multiple studies, risperidone is more likely than haloperidol to improve scores in the Positive and Negative Syndrome Scale (PANSS), reduces relapse rates of psychosis, improve cognitive function in schizophrenia, and improve manic symptoms in bipolar disorder (Salwan et al. 2013). Because risperidone has a different side effect profile than typical antipsychotic medications like haloperidol, it provides an essential alternative treatment for those people who cannot tolerate the older medications (Suh, Greenspan, and Choi 2006). The chemical designation is 3-[2-[4-(6-fluoro-1, 2-benzisoxazol- 3- yl)-1-piperidinyl] ethyl]-6,7,8,9-tetrahydro- 2-methyl-4H-pyrido [1,2-a]pyrimidin- 4-one. Its molecular formula is C23H27FN4O2 and its molecular weight is 410.49 (Salwan et al. 2013)
Risperidone is a white to slightly beige powder. It is practically insoluble in water, freely soluble in methylene chloride, soluble in DMSO (Dimethyl Sulfoxide) and soluble in methanol and 0.1 N HCl. Risperdal tablets are available in 0.25 mg (dark yellow), 0.5 mg (red-brown), 1 mg (white), 2 mg (orange), 3 mg (yellow), and 4 mg (green) strengths. Tablets of 0.25, 0.5, 2, 3, and 4 mg also contain talc and titanium dioxide. Risperdal is also available as a 1 mg/mL oral solution. The inactive ingredients for this solution are tartaric acid, benzoic acid, sodium hydroxide, and purified water (Gruber 2000).

    Background: We aimed to explore relations between symptomatic remission and functionality evaluation in schizophrenia patients treated with paliperidone extended-release (ER), as seen in a normal day-to-day practice, using flexible dosing regimens of paliperidone ER. We explored symptomatic remission rate in patients treated with flexibly dosed paliperidone ER by 8 items of Positive and Negative Syndrome Scale (PANSS) and change of Personal and Social Performance (PSP) scale. Method: This was a 12-week multicenter, open-label, prospective clinical study conducted in in-patient and outpatient populations. Flexible dosing in the range 3-12 mg/day was used throughout the study. All subjects attended clinic visits on weeks 0, 4, 8, and 12 as usual clinical practice for the 12-week observation period. Data were summarized with respect to demographic and baseline characteristics, efficacy measurement with PANSS scale, PSP, and social functioning score, and safety observations. Descriptive statistics were performed to identify the retention rate at each visit as well as the symptomatic remission rate. Summary statistics of average doses the subjects received were based on all subjects participating in the study. Results: A total of 480 patients were enrolled. Among them, 426 patients (88.8%) had evaluation at week 4 and 350 (72.9%) completed the 12-week evaluation. Patients with at least moderate severity of schizophrenia were evaluated as “mild” or better on PANSS scale by all 8 items after 12 weeks of treatment with paliperidone ER. There was significant…

    Treatment for psychosis may require some use of antipsychotic medications. These medications do not cure psychosis but instead they control the symptoms. A 1980s study showed that the medication clozapine controlled treatment resistant symptoms in schizophrenia without causing the extrapyramidal side affects that were common in the older antipsychotic medications. Since that study, there have been a number of other antipsychotic medications that have been approved for the treatment of both schizophrenia and bi polar disorder. I would support Mr. B’s decision to discontinue his use of medication for the simple reason that he has been controlling his symptoms for a long time. There would be a number of steps we would need to take. First, I would monitor the effectiveness of the medication in controlling manifestations and to identify any side affects. Although antipsychotic drugs help with managing the affects of schizophrenia, they do not protect the individual from relapse. Abrupt discontinuation of antipsychotic medication can be potentially dangerous as wells as have profound implications for long term outcomes. Individuals expressing concern about their medication should be referred to their physician for advice and monitoring. I would want to speak to their physician and e could always try to slowly get him off of the medicine as opposed to just completely taking him off the medicine from one day to the next. If Mr. B or I start to see negative schizophrenic symptoms then we would try making his dose higher again. If he continues managing his symptoms he could hopefully get off of his medication completely and be a recovering…
