Case 11-B Welcome to the Barnes City Hospital

Summary

For almost four years Paula Meers has been the night shift supervisor of nursing at Barnes city

Hospital. Paula works from 11pm-7:30am supervising twenty four nurses. Seven of the nurses

have formed a bond, inside and outside of work. These seven nurses have been with the

hospital for over 20 years. These nurses are highly respected by the medical staff and

administration. One nurse in particular is an excellent nurse with a friendly and outgoing

personality which she is also well respected by these seven nurses, Bonnie Lin.

New nurses are assigned a mentor for the first three months on this shift. New nurses are also

hazed by the group of seven women with a welcoming routine of some jokes and some non

harmful tricks. Most of the new nurses have no problem with this, they see it as a right of

passage or induction. Three of the new hires on the night shift have left after only two weeks

and Paula is concerned that this form of welcoming may be the reason. Paula decides to have a

meeting with Bonnie to discuss her concerns. Bonnie believes that with the constant pressure

and tension of caring for the patients that the jokes and tricks help lighten the mood. She also

believes any nurse that cant take a joke shouldn't be working at Barnes City Hospital, Which I

disagree. Shortly after this conversation three newly hired nurses told Paula they didn't like the

jokes and tricks treatment and resigned. They stated nursing is a serious job and they couldn't

work under these conditions. They nurses also stated they would be discussing these concerns

with the hospital administrator.

It is interesting how the media has portrayed nurses for years. Years ago the image was of a blonde, skinny female in a sexy white outfit that waits on her male only patient hand and foot. She is there to give him a sponge bath and feed him even thou it is only his leg that is broken but hands work just fine. Now that image has changed because of shows like Nurse Jackie. This HBO show is about a nurse who struggles to juggle her hectic life as a nurse at a New York City hospital. It’s based on her everyday tussles with physician other nurses and her drug addiction. Even though the show has questionable ethical circumstances it undoubtedly shows her compassionate caring nature. “Jackie is smart, caring, respected by her peers and a vigilant advocate for her patients” (Sorrell 2009). However, due to Nurse Jackie’s questionable ethical practices many nursing organizations feel that this portrayal hurts the image of nurses and hinders them from recruitment, retention, and allocation (Nemeth 2011). In which many organization have fought to refocus the community and general public’s attention on what good nursing incorporates. An organization of nurses, named The Truth about Nursing, has given awards to shows for best and worst portrayal of nursing in order to steer Hollywood to more appropriately depict nurses. Unfortunately, chaos in the media sells and I don’t believe that will change. However I am glad to see that, even thou the negative aspect of nursing as a profession are portrayed, nurses are being viewed now as compassionate, caring, smart individuals who are strong patient advocates rather than the sexy blonde who…

As far back as 1932, Hollywood in its film “A farewell to Arms” depicted nursing as having no security and autonomy. The chief nurse in the film was shown as being weak, helpless and reluctant to fight for a nurse who was dismissed because a surgeon believed that she was a distraction to his ambulance driver. In other words, physicians dictate what happens in nursing. With that coward perspective, many nurses entered into the profession answering ‘yes doctor’ to all orders without any question. Nurses are then at the receiving end of Doctors’ and patients’ bullying.…

Being that I have worked in the health care field for over eight years I have certain expectations when it comes to patient care. During my career I have been awarded several patient care awards and have had patients tell not only my head nurse manager, but the director of nursing that “the kind of care Christina has provided me is like the kind of care that I would receive from my own daughter”. I believe in caring for patients in the same manner as I would care for my own loved one.…

I believe that Judy does a great job in this aspect. I have seen her discuss a patient with a doctor and really stand up for the patient because their birthing process was being rushed for no beneficial reason. This can sometimes be nerve racking because as a nurse you have to choose between what you patient wants and what the doctor is telling you. As a nurse manager she really does have heart for what she does. “Authentic leaders care for themselves and the people they lead, and they compassion is genuine” (Marquis & Huston, 2015, p. 61). She wears her heart on her sleeve and really does love leading and caring for others.…

The delivery of healthcare has evolved and changed over the years and with the advances in science and technology, the ability to provide safe, quality care to all patients has been impacted by the changes. With the rising healthcare costs and decrease in reimbursement for care provided, organizations must look for ways of cost containment to maintain viability for the future. Fabre (2005) noted that administrators are struggling to maintain financial stability and retain competent nurses during these difficult financial times. Organizations are being evaluated and reimbursed based on their patient care outcomes; as such the nurse’s role is critical in helping to identify potential safety risks to patients and the prevention of medical errors. “As hospitals have responded to financial pressure from Medicare, managed care , and other private payers, registered nurses have become increasingly dissatisfied with the working conditions in hospitals” (Fabre, 2005, p. 20).…

During the time of my career working as an LPN, I decided to work on a Sub-Acute unit in a nursing home. The unit consisted of 30 beds and each nurse assigned 15 patients. One night during the evening shift, I noticed the other nurse administering medications in the hallway to a patient in a wheelchair. I approached the nurse and questioned if it is appropriate to dispense medications in the hallway related to the policy of the company. I further elaborated on during orientation, the preceptor expressed the importance maintaining a patient privacy and dignity and questioned if this was against policy. The nurse became very offensive and I noticed she placed something in her pocket in a hurried like manner. The nurse proceeded to explain…
