NURSING DIAGNOSIS: Anxiety related to lack of knowledge about diagnostic procedures and uncertain condition of fetus as evidenced by (S) patient states she has fears about what will happen to the baby, and (O) patient speaks with a trembling voice and exhibits restlessness.
LONG TERM GOAL/OUTCOME: Patient will demonstrate ability to reassure self and have increased external focus AEB relaxed tone of voice and affect by time of discharge.
SHORT TERM GOAL/OUTCOME: Patient will verbalize knowledge of test procedure before test initiated; couple will verbalize concerns and seek knowledge about fetus by end of shift.

Use therapeutic touch and healing touch techniques.

Anxiety was significantly reduced in a therapeutic touch placebo condition. Healing touch may be one of the most useful nursing interventions available to reduce anxiety.

    Ethical, moral, and even spiritual values inform nursing practice in a labor and delivery setting. A common nursing value, for example, is the emphasis on improving the quality of life’s experiences rather than simply “fixing problems” from a strictly medical perspective. This can have major implications for how the birthing experience is approached before, during, and after the actual period of labor and delivery, and places both the patient and the nurse in a much different total context than one that is simply medical. Besides, as a labor and delivery nurse, we also serve as patient advocates, which is enhanced by our cumulative knowledge gathered through years of experience.     Beth is a 65 year old woman of African American heritage. She was admitted to the ER, 2 days ago with a serum blood sugar of 457. She states she is unaware that she has diabetes and this is a new diagnosis for her.     For the duration of my surgical follow through experience, I had the pleasure of following RF. RF is a 49 year old male with an admitting diagnosis of cervicalgia cervical herniation, and a scheduled surgery for an anterior cervical discectomy and fusion at C4-C5 and C6-C7. According to Sharon Lewis in “Medical Surgical Nursing”, the cause of a cervicalgia cervical herniation is “the result of natural degeneration with age or repeated stress and trauma to the spine.” (Lewis, 2011) The follow through process was observed from when the patient arrived in the pre-op holding area until the patient arrived at the PACU. The expected outcome of the surgery for the patient is for the patient to be pain free within 6-12 weeks of the surgery. The radiating left arm pain should subside and the patient should be able to return to work. Immediately from the OR, the patient is expected to leave the room with immobilization of his neck as proper immobilization maintains the neck in a neutral position so there is no damage done to the spinal column during healing.     As a senior, I had the opportunity to job shadow an OB nurse at. During my time in the baby/mother area of Carilion Hospital, I experienced the admission process and discharge process of patients. I observed both mother patients and baby patients being assessed for vitals and given vaccinations. After each assessment was done, I observed my mentor as she put the information into the hospital date base program called “EPIC”. I observed 2 babies getting their hearing screened. I was also able to observe the tasks of a baby photographer, a nutritionist, and a brief consultation with a lactation educator. My mentor allowed me to listen to two heart beats of different babies; one with a murmur, the other without. I helped with simple tasks such as retrieving ice water and drinks and preparing a bed for a patient. My mentor showed me how to analyze the abundance of data on the main computer screen. This computer screen showed information such as what type of delivery was preformed (C-section or vaginal delivery), how the baby is to be fed (i.e. breastfeeding, formula), where the baby is being kept (NICU or nursery), etc. I witnessed a baby’s first bath, which can only be given after the baby is has a stable temperature. For this reason, most babies in the NICU will not have their first bath until weeks or months after they have been born. One thing that is not common that I had the opportunity to see was a patient with a nasogastric intubation (aspiration). The reason for her having this was because she had an abundance of fluids in her stomach, even after she had delivered her baby, and these fluids were not draining. Although I did not get to observe this next condition first hand due to the baby being placed in the NICU, I was informed that there was a baby born with gastroschisis. Gastroschisis is a disorder in which the baby is born with part of its intestines outside of its body, via the umbilical cord. My mentor described this…
