Late Entry

On 2/17/2016, client show up for her weekly scheduled meetings. CM along with the client complete Bi-Weekly ILP Review. At the beginning at the meeting client appears to be calm and cooperative. During the meeting client behavior changes from calm to disrespectful, rude and uncooperative. CM observed that the client shows the following behaviors------------ She continues to personalized staff as quotes” You guy are rude”.

SOCIAL SUPPORT: Client reported she does have any community support and she is not able to reside with family members.

Education: client is interested and participating in the following classes: Adobe Dreamweaver, is a 6-session, from 2/23/2016-3/10/2016 from 1pm-4pm, Social Media: An Introduction from 2/4/2016 to

She is non-compliant in meeting with onsite medical staff.

MENTAL HEALTH: Client denies any mental health issues but, this worker observed that the client display the following behavior: Client need for admiration, and lack of empathy for others. She generally unhappy and disappointed when she is not given the special favors or admiration she believe she deserve

Housing: Barrier: client is non complaint and uncooperative in participating in psychosocial evaluation to complete DHS Housing package. As per the client she prefer to bring her own but as of today client failed to submit require documents. Client also signed DHS General Pop application and DHS Psychosocial Assessment which stated client refused to participate. Client wrote on each document stating that the information is incorrect. CM also observed that the client display pathetic liar behavior.

Rules & Regulations: Client is non complaint with the Shelter Rule and Regulation and DHS Code of Conduct. CM reiterate the Shelter Rule and Regulation and DHS Code of Conduct. Client mentioned she is aware of DHS Rules and Regulation and a psychosocial is not needed to complete housing
