Case | Code | Reason for choosing that code | Removal of chalazion (stye), right eye, upper lid. | -E3 | I choose this because it describes where the stye was. | Arranged ambulance transport and extended life support | -QM | This code almost completely describes the medical scenario which is what the modifiers are supposed to do. | Diagnostic mammogram, left breast | -GG | This code refers to a mammogram which I believe will provide better detail to the medical scenario than just referring to the left side of the body. | Cortisone 10mg injection, right sholder | -RT | None of the codes discuses shots or the upper arm so the best code would be RT because it refers to the left side of the body which is where the shot was given. | Waiver of liability | -GA | Describes the waver of liability. | Intravenous catheter line, right arm | -RT | Yet again the codes that we were given did not mention a catheter line so I had to go with the side of the body that it was on. | Chest x-ray | -TC | This was the most difficult to code because none of the codes describe x-rays or the part of the body that it was performed on so I decided to go with a technical component hoping that would cover x- rays. | Prosthetic hip replacement, left side | -LT | With the codes I was give I went with the code that described what or where the surgery took place so I choose where because what type of surgery is not given in these codes.

    There are three different CPT category codes. There are category I codes, category II codes, and category III codes. Category I codes can also be known as the main codes to be used. These codes can be used by all physicians, even though they are grouped into different sections. For example, the section for Surgery codes can be used by all different physicians.
