While I conduct this Introductory Client-Intake Interview, I will think about the experiences I had when I was interviewed. By thinking of my experiences, I plan on making this interview a positive experience for the client. In the initial contact, I plan to build rapport by advising the client of whom I am and my purpose. I will then have the client to fill out Form 5.8. This form is used as a checklist for automobile accident cases. One of my tasks in conducting the interview as a paralegal is to ask relevant questions. I will only retain pertinent information about the case.
After the client has filled out Form 5.8, I will have the client information such as full name, address, social security number, marital status, phone numbers and etc. I will also retain any Related Information, Incident/ Accident Information and also a summary of the Injuries Sustained. By having the client to fill out Form 5.8, I will be able to use it as a checklist and it will ensure that I obtain all the necessary information from the client during the interview. (Miller) After the client has completed the form, I would begin asking the pertinent questions needed for this interview as followed: To begin, I would ask the client to state the name and address of her employer, her position, duties and her wages at the time of the accident.
Provide a detailed itinerary that includes the date of the automobile accident, including each place you were present, your length at each place and a list of people whom you may have seen and what you did at each place. (Model Interrogatives from plaintiff to defendent in auto accident case)
Please identify all persons that you know to have personal knowledge of the facts relating to the incident in question. When mentioning these sources, also indicate who were eye witnesses and give a summary of what the eye witness may have
