Nightingale Community Hospital (NCH) has thirteen months until their next Joint Commission audit. This report will evaluate Nightingale Hospital’s compliance in The Priority Focus Area of Communication using the Universal Protocol Standards from the Joint Commission Handbook. “The Universal Protocol was created to address the continuing occurrence of wrong site, wrong procedure and wrong person surgery and other procedures in Joint Commission accredited organizations” (Joint Commission, 2013).
The Standards of Universal Protocols (UP) are: UP 01.01.01 Conduct pre-procedure verification process
UP 01.02.01 Mark the procedure site
UP 01.03.01 Perform a Time-Out before the procedure.

To determine NCH compliance, hospital documentation was used for comparison with the Joint Commission, Elements of Performance. The following chart specifies which documents were used to show areas in need of improvement.

Executive Summary and Findings according to the Joint Commission, Elements of Performance.

UP.01.01.01 Not in compliance. #1 Implement a pre-procedure process to verify the correct procedure, for the correct patient at the correct site. The current NCH Pre-procedure list has a line item for - Identification Armband on patient’s arm; however there is no provision for procedure name, or procedure site.

#2 Identify the items that must be available for the procedure and use a standardized list to verify their availability.

    When blood is removed from a vein, it is called venipuncture or phlebotomy. Venipuncture should be performed with care. The veins of a patient are the main source of specimens for testing, the entry point of medications, and the site for IVs and blood transfusions. Because there are only a limited number of easily accessible veins in a patient, it is important that everything be done to preserve their good condition and availability. Universal precautions should be observed at all times. Gloves should always be worn when obtaining blood specimens. BEFORE BEGINNING ANY PROCEDURE—ACCURATELY IDENTIFY THE PATIENT ! Follow the proper protocol to identify the patient make positive identification of the patient by checking hospital armband for patient’s name and hospital number. Make sure the name and number correspond with the request label. When possible the patient’s identity should be verified by asking the patient to identify him or herself. Any discrepancies must be clarified before the puncture is performed. If the patient is not wearing an armband, tell the nurse you are not permitted to draw the sample until the patient is wearing an armband. Check to make the timing and other special conditions indicated for the tests ordered have been met. Personal protective equipment (gloves) should be worn during venipuncture. Be sure to use any additional protection indicated when entering an isolation room. Always wash your hands or use a hand disinfectant between patients. Make sure all the necessary equipment is on phlebotomy tray before entering the patient’s room. Item to check include: Alcohol pads, Dry and clean cotton balls, Tourniquet, Vacutainer, Collection tubes (make sure you have correct tube for all tests ordered and confirm that the tubes are not expired.) , Syringe of size sufficient to draw volume of blood needed, Safety transfer devices for use with syringe draws, Bandage tape, gauze, and/or bandages, Needles—The choice of needle will depend on the type of…
